- 304 - Repeat Premises not free of pests [s. 26(a)]
- Observation: Evidence of mice; droppings were noted in the facility. Environmental health officer observed rodent feces on the metal shelving under the cashier and large sandwich preparation refrigerator, the bottom shelf and floor area under in the lockable supply room cabinet, open faced pallet shelving in the rear single-use cups, bags, utensils and sandwich wraps shelving, and within various areas of the wall insulation. Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.
- Corrective Action: Remove any pest attractants such as food and water, eliminate access to the building and routes they use inside. Trap and remove the pests. The use of a professional pest control operator is required. Clean and sanitize area where droppings are located and monitor situation.
- 305 - Repeat Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
- Observation: Environmental health officer observed the following areas that require further repairs to prevent the entrance and movement of pests within the facility ; i) The ceiling at the front door dining area around the large metal piping, ii) the rear preparation area ceiling (above the walk in freezer and secondary residential fridge), iii) the PVC plumbing and HVAC conduit areas near the CO2/ pop dispensing area perpendicular to the hot water tank, iv) the ceiling tiles located in the very rear receiving area where the unfinished wall with exposed insulation exists, v) the gap between the wall and where the plumbing fixtures for hand wash sinks are affixed in the washrooms, vi) the exterior rear receiving door gap at the base of the door, vii) the concrete exterior wall parallel to the rear receiving door, and viii) the large crack located on the large block concrete wall to the right of the receiving door.
- Corrective Action: Operator to repair and ensure the following areas identified at time of follow-up inspection are adequately sealed to prevent the entrance and movement of pests throughout the food premises. Operator to install a door at the open doorway entrance from the rear inventory / receiving area to the food preparation / kitchen area to help further prevent pest movement to the front of the building from the back.
- 312 - Items not required for food premises operation being stored on the premises [s. 18]
- Observation: Environmental health officer observed various items not required for the food premises operation being stored on the premises such as; three sets of automobile tires, a large pile of miscellaneous non functioning computer and office equipment within the rear receiving area of the facility. These items permit the movement of pests into walls, shelving, ceiling piping and wall openings at service conduits and, creates an area where pests can harbour / nest within the facility.
- Corrective Action: Operator to remove miscellaneous items that are not required for food premises operation from the facility (i.e automobile tires, old food service and office equipment observed within the rear receiving area).

- 201 - Critical Food contaminated or unfit for human consumption [s. 13]
- Observation: (CORRECTED DURING INSPECTION) Contaminated foods on premises. Several food items were discarded at time of inspection by the environmental health officer. The following items were discarded to waste: 52 Italian Herb and Cheese Cooked Loaves, 19 White Bread Cooked Loaves, 10 Multigrain Cooked Bread Loaves, 1 Package of Opened White Wraps, 7 White Ciabatta Buns, 70 Raw Italian Herb and Cheese Raw Pre-Proofing Loaves, 20 Brown Raw Pre-Proofing Loaves, 30 White Pre-Proofing Loaves and one large bag of purple onions. (Public Health significance) - All foods on the premises must be wholesome and free from spoilage or adulteration and otherwise fit for human consumption. Unacceptable food items could cause food borne illness and become insect and rodent attractants or inadvertently be added to other food.
- Corrective Action: All foods that are spoiled, adulterated or otherwise unfit for human consumption were observed to be discarded to waste and removed from the premise at time of inspection.

- Observation: (CORRECTED DURING INSPECTION) Improper thawing procedure, foods thawed sitting at room temperature. Observed 6 prepackaged bags of roasted chicken, 2 packages of beef steak and 1 package of grilled chicken strips thawing at room temperature. Environmental health officer temperature checked the thawing meat products and a temperature of 22.6C was observed at time of inspection. Environmental health officer inquired with the employee about how long these items were left at room temperature and the employee was unsure. The environmental health officer directed the employee to discard the temperature abused meat products into the garbage and remove from the premise at time of inspection. (Public Health Significance) Thawing at room temperature is uncontrolled and over the required time, portions of the product can be in the danger zone for extended periods of time.
- Corrective Action: Thaw foods under running cold water, in the fridge or in the microwave oven.

- Observation: Evidence of mice; droppings were noted in the facility. Environmental health officer observed numerous areas and food contact surfaces with rodent feces, and observed both dead and alive rodents in pest control glue traps at time of inspection. (Public Health Significance) Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.
- Corrective Action: Remove any pest attractants such as food and water, eliminate access to the building and routes they use inside. Trap and remove the pests. The use of a professional pest control operator is recommended. Clean and sanitize area where droppings are located and monitor situation.

- Observation: Waste removal and cleaning schedule inadequate for the volume produced. Environmental health officer observed a heavy accumulation of food waste in all waste receptacles at time of inspection. Environmental health officer observed rodent feces within the waste containers at time of inspection.
- Corrective Action: Pick-up frequency, cleaning schedule of dumpsters and containers must be adequate to prevent a nuisance and being a food source for pests.

- Observation: Environmental health officer observed a heavy build up of old food debris, food splatter, dust and grime on floors under equipment, preparation tables, and wall surfaces within the front counter / sandwich food preparation and kitchen preparation area and rear dry inventory storage area. The accumulation and build up of old food debris, dirt, grease and grime create a food source for pests, permit the harbourage of pests within the facility can contaminate food and food contact surfaces which can cause potential food-borne illness and / or disease.
- Corrective Action: Operator to remove all old food debris, dirt, grime from the floor, wall and food contact surfaces, and then thoroughly wash and sanitize to prevent cross contamination, harbourage of pests and maintain a clean and sanitary environment.

- Observation: Observed tubular florescent lights without plastic safety casings over food preparation area. Broken glass from the lighting source without safety covers / casings permit a direct physical hazard of glass contamination into food products.
- Corrective Action: Operator to purchase, install and ensure that all tubular florescent lighting bulbs in the premise, and especially in food preparation areas are properly secured and covered with the appropriate safety barrier casings.

- Observation: Observed improper thawing procedure(s) of various chicken and beef food products on rear stainless steel food preparation table sitting at room temperature. (Public Health Significance) Thawing at room temperature is uncontrolled and over the required time, portions of the product can be in the danger zone for extended periods of time.
- Corrective Action: Thaw foods under running cold water, in the fridge or in the microwave oven.

- Observation: Within the sandwich food assembling area, food utensils were observed to be heavily soiled within the green sanitizing solution holders / containers. Staff verbally indicated that the santizing solution holders did not contain the adequate and appropriate sanitizing solution.
- Corrective Action: Operator to ensure staff properly understand, are trained and adhere to all corporate sanitation protocols and procedures within the premise to reduce the risk of pathogenic organisms harbouring and growing on food utensils.

- Observation: Observed initial stages of bacterial biofilm accumulation on large stainless meat slicer along circulating blade.
- Corrective Action: Operator to ensure staff properly understand, are trained and follow all corporate sanitation procedures within the premise to reduce the risk of pathogenic organisms harbouring and growing on food service equipment.
- 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
- Observation: No sanitizing solution or detergent for the dishwashing equipment was observed at time of inspection. (Public Health significance) - Automatic dish washers must provide a minimum concentration of 50 parts per million (ppm) as required to effectively sanitize dishware, 100 ppm recommended; Hand washing requires 200 ppm unscented bleach solution.
- Corrective Action: Immediately initiate hand sanitizing providing a 200 ppm soak for 2 minutes for all dish ware. Have the dishwasher repaired by a qualified technician and monitor and log sanitizer concentration regularly so you know when the unit needs adjustment or repair Operator to contact health officer once the dishwasher is repaired and is adequately supplied with the appropriate chemical sanitizing solution.

- Observation: Evidence of rodents; droppings were noted along the floor along the interior walls in the rear receving, dry inventory storage area and front-end food service area of the facility.Observed rodent feces on the floor area next to the cashier and along the backside of the parallel wood shelving below the refrigerated sandwich assembly area where single service sandwich wraps are stored. (Public Health Significance) Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.

- Observation: Observed large opening between the bottom of the lockable premise rear receiving door to the floor that can permit the entry of rodents and other pests within the food premise. Observed large unsealed gaps on the interior walls where conduit and plumbing service openings are present. Openings or gaps within walls can permit harbouring and breeding of pests within the premise and permit movement of pests from rear storage area to the front food preparation and food service area which can contaminate food contact surfaces, utensils, and food products.Observed various organic food debris on premise storage floors, under rear food preparation area and dishwashing area.
- Corrective Action: Operator to purchase and install weather stripping or sealent to close large gap opening on the rear premise receiving door to prevent the entry of pests from the exterior in the food premise. Operator to repair and / or seal all interior wall gaps around electrical conduit and plumbing service openings to inhibit and prevent pest movement, harbouring and breeding within the premise. Operator to thoroughly clean and remove all food debris observed on floors and counters to reduce pest food resource and presence
