      • **** No diapering is performed in this facility****There was a bottle of sanitizing solution that measured 0 ppm to QUATS test strips. The staff provided a container with a concentrate that is used to make the sanitizing solution labeled Formula 333, instructions on contact time was not indicated on the concentrate bottle. The staff made a mixture of 200 ppm quat solution in the spray bottle during inspection. Ensure to provide manufacturer's instructions for use to verify minimum contact time.
