      • Miscellaneous decorative items and supplies were stored in a crib accessible to children.Please remove the items from the crib.

      • 1) The ceiling grates throughout the facility were dusty.Please clean the grates to ensure they are free of dust and loose debris.2) The paint from the door trim leading into the infant room was observed to chipped, exposing raw wood.Please paint the door trim to ensure the surfaces are smooth, easily cleanable, and impervious to contamination.

      • 1) The carpet located in the infant room was observed to soiled with food stains, dirt and debris.Please deep clean the carpet.2) The tops of the storage units located in the infant/toddler room were observed to be worn, exposing the raw wood underneath.Please repair the tops of the storage units to ensure the surfaces are smooth, easily cleanable, and impervious to moisture.

      • Cardboard was used as a liner between the mattress and the slats. Cardboard is not a smooth, easily cleanable material that could potentially trap moisture and contaminants.Please remove the cardboard completely and use only the slats, or replace the liner with one that is smooth, easily cleanable, and impervious to moisture.

      • A bin of beads were stored on a lower shelf located in the infant room by the exterior door, easily accessible by children.The operator promptly reorganized the shelving unit to ensure that small items, such as beads, are stored out of infants’ reach.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • The first aid kit was missing a CPR mask and tweezers.Please obtain a CPR mask and a pair of tweezers.

      • Dirt, crumbs, and loose debris were observed underneath the diaper changing mat.The operator promptly cleaned and disinfected the surfaces under the diaper changing mat.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Gate used to partition the infant space from the kitchen was in disrepair. Proper prevention of kitchen access was not available. Repair or replace partition gate.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Diapering table was not disinfected between changing clients. The diapering table must be saturated with 100ppm bleach with a contact time of 1 minute between each client. 2. Staff did not wash their hands, or the clients hands after diaper change.Both the clients hands and staffs hands must be washed after conducting a diaper change.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1. No paper towel in childrens washroom located by the playground exit door. Paper towel restocked during inspection. 2. Paper towels at diapering station were not stored in a dispenser. Paper towel stocked into proper dispenser during inspection.

      • 1. Diapering table surface in disrepair. Guard rail and wooden surface paint is chipping, exposing raw composite wood. Repair finishings of diapering table. 2. 4 compartment shelving unit by front entrance door had a styrofoam backing. Styrofoam backing was in disrepair. Repair or replace the backing of indicated shelving unit. 3. Tape holding up the diapering station handwashing sink mirror is in disrepair. Permanently secure the mirror to the wall. 4. Backs of the green preschool chairs were visibly dirty. Clean indicated area.5. Underneath the diapering pad was an accumulation of dust and debris. Clean and disinfect under the diapering pad frequently.6. Infant crib was filled with miscellaneous storage materials. Remove items from infant crib.

      • Infant bedding was stored on top of made bed in infant crib. All bedding must be stored seperately with no contact.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • No toilet paper dispenser in the children's washroom by the playarea exit. Provide a proper toilet paper dispenser.

    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The safety gate separating the classrooms was not secured to the door frame. The bottom portion of the gate was loose.Please repair or replace the safety gate to ensure it is secured to the door frame.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • 1. One of the shade blinds were in disrepair. The bottom rail for the blind was sticking out. The operator removed the blind during inspection.Please repair or replace the shade blinds. Ensure there are no blind cords dangling down.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1. The staff washroom is cluttered, and equipment are stacked on top of each other. This can be a potential safety hazard for staff.The operator stated some of the equipment will be removed from the washroom.Please organize the space.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • 1. The sanitizer spray bottle located in the diapering area was in disrepair. The sanitizer bottle was changed out during the inspection.

      • Alcohol based wipes were expired in one of the first aid kits.- Staff removed the wipes and have replacements to stock up.

      • Small tear in diapering change table material.- Staff replaced the damaged change table material.
