      • Observation: Ceiling tile still not painted or replaced as noted on previous inspection.
      • Corrective Action(s): Discussed with operator and he has committed to tackling this issue (at least to get started) by next scheduled routine inspection.

      • Observation: Sushi bar area as well still in a rather deteriorated state as per last inspection. Operator stated due to time constraints he has not finalized his plans for the front bar area construction.
      • Corrective Action(s): Monitor. Operator needs to get a game plan together as noted in previous inspection.
      • Review status at next scheduled inspection.

    • 307 - Equipment/utensils/food contact surfaces are not of suitable design/material [s. 16; s. 19]
      • Observation: Ceiling tile (accoustic) in kitchen is not the correct material to allow easy cleaning. Air exhaust/intake in back area is quite dirty and dirty cobwebs developing.
      • Corrective Action(s): Develop plan to replace ceiling tiles with proper washable food grade tile in kitchen area. Start in area where tiles are dirty and expand to enitre kitchen space. imeline 6 months to one year.

      • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: A) shelving in kitchen area deteriorated and flaking wood.
      • B) cupboard below sushi bar deteriorated again. Likely due to heavy water use in area and the repaired wood eventually allowed water to absorb into wood. Wood bugs noted in significant numbers in this area.
      • Corrective Action(s): A) Remove deterioed material as discussed.
      • B) CLean up this space and remove old and deteriorated material. Operator mentioned his plan is to replace the sushi bar with updated material and finishes.
      • Review at next scheduled inspection.

    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: Shelving unit next to cookline (particle board surfaces) severly deteriorated and is flaking wood and mouldy. Surfaces not cleanable. This was observed from previous inspection.
      • Faucet at back double sink is leaking and has been wrapped in plastic. The wrapping and leaking makes these sinks and space difficult to clean and sanitize, and may be contributing to the leaks to extend to the grease trap, which is again beginning to rust. Continued leaking of water in this area will cause the grease trap lid to deteriorate in an excelorated manner. Word of caution.
      • Corrective Action(s): Due to the possibility of the wood particles contaminating food in this area and the inability for these surfaces to be cleaned/sanitized, all deteriorated wood surfaces in this area must be immediately removed from premises. Replace at your choice.
      • Fix the sink faucet as discussed.
      • Review of both issues to be reviewed at follow up inspection date noted. Send pics of completed work.

    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Mouse droppings observed in select areas of premises, most noteable around the front bar cabinetry. Droppings also noted in back area around containers on the floor by mop sink.
      • Corrective Action(s): Make sure to keep food residues to a minimum to control food sources for pests. Droppings may mostly be artifacts of activity and not current activity, but they need to be cleaned up to be sure and for you to target control efforts. Continue to monitor and continue to have pest control in place.

      • Observation: Significnat clean up has occured, but some mouse droppings still noted. To be expected.
      • Corrective Action(s): Continue to trap and block off entry points. Continue to keep things cleaner and better organized and be sure to follow Integrated Pest Management practices (clean, kill, control entry).

    • 205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
      • Observation: Sushi bar cooling unit measured 8.5 C with probe thermometer and measured up to 10 C with IR gun.
      • Corrective Action(s): Immediately have unit serviced by technician. Implement food rotation practices in this unit such that all food is replaced within two hours and/or discarded as per food safety plan. Smaller volumes of food must maintained in this unit.
      • **Make sure when thawing food not to leave the food in a location that cannot be monitored/observed. If allowing food to thaw, best practice should be in cooling unit, not at room temperature (crab meat), and times should be monitored.
      • **Check at follow up inspection date noted.

    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Significant mouse infestation observed. Dropping on ground in multiple areas of kitchen. Most concentrated in back area of kitchen including: Along dry food storage shelves; at hot water tank and mop sink area; Front sushi bar area;
      • *Droppings and activity likey in other hard to reach areas including behind equipment, under equipment, in corners and near access points like plumbing and electrical conduits. Sticky traps placed by operator and two mice noted stuck.
      • Corrective Action(s): Immediately contact Pest Control Operator (PCO) and enter into contract for services. Seek advice and recommendations. Implement recommendations by PCO, utilizing integrated pest management practices.
      • Follow up inspection to review contract and recommendations as noted on report.

      • 306 - Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Back area of premises, bar area of premises and cookline area of premises not cleaned on regular basis in a manner to prevent build up of food debris, greases and mouse activity (droppings). Hard to reach areas behind and under equipment is not being included in regular cleaning activities. Sanitation plan is not being implemented or followed. Operator is not verifying work is being completed.
      • Corrective Action(s): The entire premises requires a thorough clean up and this clean up will be audited on follow up inspection noted in report. Equipment will be moved to audit the quality of the cleaning done by staff.

      • Observation (CORRECTED DURING INSPECTION): Sushi bar cooler and food product inside noted between 6 and 9 C. Doors possibly left open. Compressor unit very dusty and dirty.
      • Corrective Action(s): Monitor unit and service if necessary. Must maintain 4 C or less, and should be closer to 0 C due to nature of foods stored inside. Clean compressor unit, motor, and area surrounding unit. Will review at follow up inspection date noted.

      • Observation: Raw proteins stored directly above cooked or ready to eat foods in many of the cooler units. Discussed.
      • Corrective Action(s): Re-organized cooling units so raw proteins are stored below ready to eat foods or in alternate storage areas in cooler units.
      • Review at re-inspection date noted.

      • Observation: Extensive use of cardboard barrier in multiple food storage areas including coolers as means to avoid cleaning. Cardboard is not an acceptable surface and does not promote easy cleaning, and as such could be a source of contamination of food or food containers.
      • Corrective Action(s): Discontinue the use of cardboard as surface barrier. An exception is allowed for the deep fry station, as long as cardboard changed daily and is not used as a direct or indirect food contact surface. Review at follow up inspection date noted on report.

      • Observation: Cupboard/cabinetry space below sushi bar sink/counter and surrounding sections significantly deteriorated. MDF is rotted, flaking and the unit is falling apart.
      • Black carbon and possibly mould is building up in these areas. Dust and cobweb accumulations significant.
      • Pictures taken of this area.
      • Corrective Action(s): Immediately clean up this area and remove any equipment, utensils and/or food containers from these areas. Clean up the entire sushi bar area. To be reviewed at follow up inspection date noted on report.
      • Develop a plan to repair and/or replace structure to allow this space useable for food prep activities. Have plan to this office within 30 days.

      • Observation: Some areas in the kitchen observed with food accululations (floor) or food splatters (walls). Discussed with operator to update scheduled cleaning practices to include:
      • - Daily wipe down of walls near high activity food prep areas i.e. Below pass through wall, at rice prep station wall.
      • - Floor under shelves along wall look pretty dirty when shined with flashligh. Move shelves and clean. Add to weekly or bi-weekly schedule.
      • Corrective Action(s): Clean as discussed. Move equipment periodically and containers on the floor frequently to ensure cleaning process is complete.
      • DO NOT USE cardboard as surface to replace cleaning. OK to use in high grease impact areas only if it is changed daily.
      • No on shelves. If shelves are old, replace. Some are old.

    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: 1) Prep counter and sink area deteriorated. MDF (particle board) exposed in several areas and is a risk to flake off into food. Water is leaking from around sink surround down into cupboard below. Back splash is duct taped to wall and although the repair has removed some of the old repair, mould and old food debris continues to accumulate around faucet. No contact made with this office as requested from previous inspection. Pictures taken.
      • 2) Grease trap and pipe network around back sinks severly deteriorated and rusted. Grease, food debris and waste water from leaks has accumulated to a point that this area is not able to stay sanitary and is potentially a health risk. Pictures taken.
      • Corrective Action(s): 1) Repair countertop properly as requested multiple times. A correction plan must be submitted and approved prior to commencement of work.
      • Written plan or plans for counter to be submitted to this office by August 1, 2019. Work to be completed by September 15th, 2019.
      • 2) Repair grease trap and all plumbing pipe work around grease trap and back sinks. This work must be completed by a certified plumber and must conform to the BC Plumbing Code.
      • Written plan for grease trap and surrounding plumbing work to be submitted to this office by August 1, 2019.
      • Work to be completed by December 1, 2019.

      • Observation: Work not performed on either the grease trap or the counter as noted from previous inspection. Discussed with operator.
      • Corrective Action(s): Counter top and back splash to be prioritized for reconstruction work. Operator to complete within 2 month period and will email to confirm the work details. Make sure corrective work plan is a long-term solution.
      • **Grease trap is severely rusted, but not a direct or indirect food storage environment. no longer used for equipment storage. Reconstruction work can be extended to longer term (6 months to 1 year), but watch for leaks as this is waste water and would pose a serious health hazard if the grease trap contents got into the kitchen area. Monitor closely.

      • Observation: Build up of food debris noted in several areas of premises. These include:
      • - Bottom of display cooler;
      • - floor below and around large containers of liquids adjacent to display cooler;
      • - Behind and around garbage bins and equipment in back of premises.
      • Corrective Action(s): Clean up these areas. Remove all cardboard from cooling units. This will facilitate better cleaning procedures in this unit and help air flow to all parts of cooler.

      • Observation: A) Countertop at double sink and handsink in food prep area significantly deteriorated and has been patched many times. Arborite pulling away from wood underside. Mould and slimes building up. This countertop is no longer viable as a food contact surface.
      • B) Grease trap lid in extremely poor condition. Rusting away and virtually impossible to clean. Water is leaking from the drain pipe above. Water is stagnating on top of trap lid.
      • Corrective Action(s): A & B) Countertop must be replaced. Grease trap lid must be replaced. Any leaks originating from the taps or drain pipes must be repaired. This work must be completed by next scheduled inspection.

      • Observation: Cooling units observed with old cardboard on bottom of unit. Door handles on same observed with minor build up of hand prints. Contamination points. Food debris and related build up noted.
      • Also food debris noted in freezer unit.
      • Corrective Action(s): CLean up noted areas as discussed. Discontinue use of carboard in coolers as tool to avoid cleaning. Use in dry food only if they are replaced daily. Should not substitue daily regular cleaning schedule.

      • Observation: Area behind and below the grill and deep frier observed accumulating lots of grease and food debris. Also, area behind the deep freezer doing the same. Also, some walls in food prep area especially around work surfaces observed allowing the accumulation of food debris.
      • Corrective Action(s): Thoroughly clean up these areas and ensure cleaning frequency and/or quality better maintained.

      • Observation: A) Several areas of the kitchen including work surfaces and storage shelves deteriorating and must be removed and replaced:
      • - Shelves beside grill. These are flaking wood and wood-like finish and have become impossible to properly clean and sanitized.
      • - plywood work, cutting or prep surface (discussed with operator).
      • B) Areas of the kitchen beginning to fall apart and a plan should be considered for future correction and replacement. This includes:
      • -Countertop at the prep and hand sink.
      • Corrective Action(s): Correct (A) by follow up inspection date. Monitor (B) for long-term fix. Discussed with operator.
