- Did operator properly sterilize reusable equipment?
- Load sterilizer according to manufacturer's instructions.
- Mechanical, Chemical and Biological Monitoring: Did operator follow directions of a medical officer of health or public health inspector?
- Check and document the following parameters: the actual time of the sterilization phase; the actual temperature during the sterilization phase; the actual pressure reached and maintained during the sterilization process.
- Process a biological indicator according to manufacturer instructions.
- Ensure the spore test is contained in a process challenge device. There must be a full load in the sterilizer being tested.
- Process challenge device: A test device intended to provide a challenge to the sterilization process that is equal to, or greater than, the challenge posed by the most difficult item routinely processed. Examples include, but are not limited to, biological indicator test packs which also contain a chemical indicator. (Show More)
- Ensure that the service provider performing biological indicator testing is able to: provide test results electronically within 48 hours of incubation, immediately communicate a spore test failure to personal service workers and confirm receipt of information to a live person; and maintain records that include: date of spore test, date of incubation, date of reporting the result, worker who conducted the test, unique identifier/lot number printed on the spore strip package and the growth medium. (Show More)
- Follow-up with the laboratory to obtain biological indicator results in a timely manner.
- Do not use processed equipment until after the biological indicator results have been received and are negative i.e., passed.

- Mechanical, Chemical and Biological Monitoring: Did operator follow directions of a medical officer of health or public health inspector?
- Mark the sterilization date and, if used, the lot number, on the packaging (beyond the sealed edge) after reprocessing.
- Ensure each sterilization package has appropriately placed chemical indicators. Two indicators are required; inside (minimum Type 4) and outside (Type 1). Packages with built-in indicators are recommended.
- Use a Type 1 external chemical indicator (e.g., tape, label) on every package that is processed in the sterilizer.
- Use a Type 4 internal chemical indicator (e.g., paper strips) inside every package that is processed in the sterilizer.
- Challenge the sterilizer once every 14 days at a minimum with a biological indicator (e.g., spore test).
(Show More)
- Send biological indicator (e.g., spore test) to a service provider capable of performing spore testing and certified to recognizable standards (e.g., International Standards Organization).
- Follow-up with the laboratory to obtain biological indicator results in a timely manner.
- Do not use processed equipment until after the biological indicator results have been received and are negative i.e., passed.
- Include a biological indicator in every load containing implantable devices. Implantable devices shall not be used until the results of the biological indicator are available.
- Mechanical, Chemical and Biological Monitoring: Did operator follow directions of a medical officer of health or public health inspector?
