    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • The OSC washroom sink was not working at the time of inspection. The mixer valve was said to be leaking. Repairs have been scheduled. The sink just outside of the kitchen was being used as a handwash sink until the OSC washroom sink has been repaired.

      • Chlorine solution measured 500 ppm. Operators requestion to ensure concentration is 1000 ppm until outbreak declared over. Sheets for sleeping are being kept on cots and then labeled with each child's name. It is suggested to remove sheets, launder between use, and wipe cots with chlorine solution between use.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • Antiseptic wipes for the first aid kit were expired. Have staff check all contents of first aid kit and ensure they are fully stocked. First aid kit content list provided to operator via email.

      • Art supplies in various rooms were not labelled as non-toxic or were labelled for children older than 6 years. The supplies included but were not limited to:- Paint.- Glue Sticks.- Bingo dabbers. Remove items from use.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Baby room hand sinks was being used to store soiled lunch items. Hand sink is to only be used for the purposes of handwashing and kept clear at all times.The items were removed once noted.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • Antiseptic wipes for the first aid kit were expired. Have staff check all contents of first aid kit and ensure they are fully stocked. First aid kit content list provided to operator via email.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • A wooden climbing structure has a height greater than 20 inches. Ensure that proper fall surfacing and a 6-foot encroachment zone is available for fall protection.PHI suggested lowering structure if possible or not using for climbing.

      • *Walls in disrepair. Areas were observed to be needing repair. Exposed drywall and paint peeling of the walls was observed.*Door to washroom for the OSC area appears to be in disrepair. Ensure all areas are easily cleanable and impervious to moisture.

Ensure all equipment is in good repair.*Exposed wood for the OSC cubbies was observed.       • *Washroom vents appear to be dusty. No cleaning records were observed. Please ensure cleaning is done as needed.*Toilet not working in the toddler room. Ensure all sanitary water closets are in good function.

      • In both toddler's room, handwashing was being completed by handing the kids wet towels dipped into water and soap solution in a bucket.Ensure kids complete proper handwashing steps under running water every time hand washing is required

    • 06. Are proper protocols being followed in regards to client care?
      • There was no safety belt for the diapering mat placed on an elevated diapering bed in the Baby room. The operator stated that a safety belt was on order. Please ensure the diapering surface is supplied with a safety belt.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • A brown chair in the Kinder/OSC room had torn edges with exposed porous material, making it not easy to clean.The staff present removed the chair during inspection.

      • ***Chlorine solution was tested with chlorine strips and the concentration was observed to be approx. 400-500 ppm.Please ensure a stronger concentration is used in an outbreak situation. Ensure test strips are available to test concentration of the solution.

      • It was observed that the surfaces and edges of the cubbies were not smooth and impervious to moisture. Ensure all areas are easily cleanable and impervious to moisture.
