      • Paper towels used for handwashing in the room on the lower level were not in a dispenser. Please obtain a paper towel dispenser.

      • The diapering mat was dirty. Please replace the diapering mat and ensure that both the top and bottom and cleaned and sanitized after each use.

      • Tape was observed on the play stove in the room on the lower level. Additionally, tape was observed along the side of the countertop in the same room. Please remove the tape on the play stove and the countertop. All surfaces, toys and equipment must be smooth and easy to clean.

Please ensure the cord is not accessible to the children.4. There is a fire exit door in the room that can be opened by the children. This is a safety hazard. Please ensure the children do not have access direct access to the fire exit.5. There is a wooden cabinet with openable drawers. Please ensure children do not have access to these drawers.
