      • An incomplete vendor list was provided to AHS. Several special event vendors were identified as not having provided notice to AHS of their operations. Ensure that a complete list of vendors, including mobiles, is provided to AHS when submitting forms for special events. Ensure that vendors are submitting special event vendor notification forms. Vendors with mobile permits need to submit forms if they are operating outside the scope of their mobile permit restrictions (i.e., a pre-packaged only vendor preparing samples onsite).
