- The ceiling vent cover located above the sandwich prep counter/cooler in the front food handling area was cracked and damaged at the time of the inspection.Sections of the vent were noted to have been taped to secure the vent to the frame, and multiple sections of the plastic vents have been broken, and sections of plastic are missing.This condition poses a physical contamination risk of the sandwich prep area and ingredients.Ensure that the damaged vent is repaired, replaced or altered so that it is made to be in good repair, and so that foods and food preparation areas are not at risk of physical contamination.The facility has indicated that custom replacement options and retrofitting options are currently being considered for implementation.

Operator noted that this is scheduled for repair.ACTION: please replace/repair the flooring so that it is smooth, durable, non-absorbent, easy to clean. - Under the front-facing hand sink near the entrance to the food handling area, there was a square piece of drywall-like material leaned up against the sinks plumbing fixtures. This material had visible water damage, was not durable or non-absorbent, and had mold-like growth.This board was being used as a barrier to block off the area under the sink. It was being contacted with water during regular floor mopping.This board was discarded by the operator.ACTION: please ensure that surfaces are smooth, durable, non-absorbent, easy to clean.

There is a catch-basin under the oven for the water which is insufficient given the observed pools of water under the donut station table next to the oven.Pooling water can lead to unsanitary conditions such as mol and pests. It can also pose a slipping hazard. Operator stated that this will be addressed promptly.Please resolve the source of the leaking water.

- High temperature dishwasher turned off and on throughout the day, disrupting the machine's ability to reach sanitizing temperatures when run. Dishes being washed during inspection. Plate level temperatures measured from 51C to 66C when machine was still warming up.Please refrain from turning the dishwasher off and on as it disrupts the ability of the machine to reach sanitizing temperatures.The dishwasher can either be left on once turned on for the day, or it can be left off until dishes need to be done. Manual wash-rinse-sanitizing in the 3-compartment sink can take place when the dishwasher is off or warming up.Dishwasher to be tested by PHI again this week.
