- The operator failed to notify AHS of the addition of a new service (head spa) to the personal service business.-Ensure AHS is notified of the addition of any new services to be rendered in order to review your process.

- The written procedures for the facility were not available on site. Ensure the following written procedure are available onsite at all times: -Client care in case of an accidental cut-Preparation and cleansing of skin-Application of antiseptic-Decontamination of dual use sink-Post care instructions

- Porous single use tools were noted to be reused in the facility. The following were noted during the inspection:-Stained single-use toe separators were stored in carts in the pedicure station to be reused-Dirty Single use lashes spools were stored in storage cups in the lash room for reuse-Dirty/Used nail buffer was stored alongside clean tools in a ready to use customer package in the clean tools' cabinetRequirement-Discard all used porous tools as tools cannot be adequately washed and disinfected-Ensure that all single use items are used in the manner they were intended for.

- Used dirty drill tips were kept with clean drill tips after attending to clients.-Ensure dirty and clean equipment are separated and cleaned appropriately.

- The pillows in the room used for head spa were made from porous material that cannot be adequately cleaned after each use-Remove pillows from facility or cover with easily cleanable, nonabsorbent material that can be properly cleaned and disinfected after each use.

- The pedicure basins were not properly cleaned and disinfected. Staff on site stated Lysol spray is applied to bowl, then water is added and scrubbed down before rinsing.-Ensure that proper procedure for cleaning and disinfecting basins are followed-Wash basins with soap and water, rinse, then apply a minimum of low-level of disinfectant. -Allow disinfectant to sit for the appropriate contact time before final rinse

- 1. Improper disinfection of reusable tool was noted during the inspection. The foot scrapers stored in the clean tools' cabinet, cannot be fully immersed in the disinfection tray. The tray was too small to fit the tool.-Ensure that the disinfection tray used in the facility is large enough to fit all tools that are to be disinfected.-Ensure that reusable tools are fully immersed in disinfectant solution when reprocessing.2. Dirty tweezers used for application of lash extension were stored in dirty storage cups in the lash extension room3 Dirty lash molds were stored in dirty storage bins in the lash extension room-Ensure that dirty/used semi-critical tools are transported to the reprocessing area to be cleaned and disinfected immediately after each use.-Store disinfected tools in a clean and sanitary manner to prevent contamination of tools4. Staff on site reported that nail drill bits are soaked in alcohol to disinfect.-Alcohol is not a disinfectant**Nail drill bits (non-critical tool) can be disinfected using a low-level disinfectant e.g. Lysol wipes

- Some bottles in the facility (In the room used for lash extension and at the pedicure station) were unlabeled.-Ensure all cosmetic used in the facility are clearly labelled to identify its content.

- Staff on site reported that contact surfaces in the facility were disinfected using alcohol. -Alcohol should not be used as a disinfectant. All surfaces in the facility must be disinfected using at least a low-level disinfectant.

- No paper towel was found in the hand washing station at the Head spa room and lashes room. Single use reusable hand towel was provided.-Ensure there is always sanitary options for drying hands like clean hand towels, paper towels or air dryer.
