    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • The facility did not have written procedures for skin cleansing prepared. Technician was provided with an AHS template. Please ensure the template is filled out and stored on-site.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • An approved anti-septic was not being used on skin prior to piercing. A product must be labelled as an anti-septic and be labelled with a DIN to be an approved anti-septic.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • The facilities piercing gun is a plunger-style gun with no disposable parts that cannot be taken apart for proper cleaning. Operator was informed of alternative options for acceptable piercing guns.
    • 20. Is reusable equipment disinfected properly?
      • The facility was using a product without a DIN or disinfectant claim to disinfect semi-critical equipment (plunger-style piercing gun). A high-level disinfectant must be used for disinfecting semi-critical equipment.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • The earrings used for piercing do not have a sterility claim on the packaging. Critical equipment used to pierce the skin must be sterile at use.
