- Completed the written procedures at time of inspection except missing written confirmation regarding the washroom sink for reprocessing purpose.PLEASE PROVIDE PHOTO EVIDENCE THAT THE WATER FOR THE TOILET AND SHOWER IS SHUT OFF. PROVIDE WRITTEN CONFIRMATION THAT THE TOILET AND SHOWER WILL NOT BE USED IN THE BASEMENT WASHROOM AS ITS SINK WILL SERVE SOLELY FOR DEVICE REPROCESSING AND HAND WASHING PURPOSES ONLY.

- Tweezers are disinfected in UV storage unit and alcohol. UV AND ALCOHOL ARE NOT APPROVED DISINFECTION METHOD. PLEASE DO NOT USE.TWEEZERS FOR EYE LASH EXTENSION MUST BE SCRUB WITH SOAP AND WATER, THEN RINSED, THEN SOAKED INTO A HIGH LEVEL DISINFECTANT FOR THE REQUIRED CONTACT TIME (AS PER MANUFACTURER'S INSTRUCTION FOR USE) IN AN APPROPRIATE LABELED CONTAINER.

- Operator uses an unfinished wooden stand as stepping stool for client to get onto the chair to provide the service. The chair is also placed on an unfinished wooden stand. PROVIDE WATER RESISTANT CLEANABLE SURFACE OF THE WOODEN STANDS TO FACILITY PROPER REGULAR CLEANING.
