    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures are not complete. Fill out AHS written procedures template and send via email.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Handwashing sink in facial room was not equipped with soap. Ensure that all sinks designated for handwashing is fully supplied with soap, paper towel, and hot running water.
    • 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
      • Cleaned equipment was being stored in foam lined containers with no barrier or protection. Store all clean equipment in smooth, non-porous containers at all times.
    • 20. Is reusable equipment disinfected properly?
      • 1. 70% rubbing alcohol used in facility did not have a DIN or NPN number available at the time of inspection. Ensure all disinfectants and products have a DIN or NPN number. 2. Athena speculum is not being adequately disinfected. Semi-critical tools must be disinfected with a high-level disinfectant. Obtain the manufacturers instructions for use for this product. 3. Non-critical surfaces such as beds and countertops are not being disinfected between clients. All non-critical surfaces must be cleaned with at minimum a low-level disinfectant between client use.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. Hand sanitizer and Accel disinfectant are being improperly topped up. Ensure that all products are properly topped up by emptying vessel, cleaning, and drying before refilling. Correlating expiry dates must be recorded on bottles. 2. Single use vials were being used as multiuse vials. Manufacturers instructions for use, discard remaining content after use, were not being followed. Follow MIFU at all times3. Botox was being reconsituted with a preservative Saline. Botox MIFU states that a preservative free 0.9% sodium chloride injection USP.
