- 1. Public Health Inspectors were verbally refused to enter and inspect by the receptionist because no appointment was made and that the owners were not present. Please note that an Executive Officer / Public Health Inspector may enter any public facility during their hours of operation to conduct an inspection without appointments nor owners to be present.

- December 18, 2023 inspection:Manufacturer's Instructions for Use was not available for the Eppendorf centrifuge. Please obtain MIFU and have it readily available for reference for staff. Angela was unsure about the MIFU in regard to cleaning and disinfecting the centrifuge. The device is currently disinfected with 1:10 bleach diluted solution.Feb. 28, 2024 re-inspection: Ramel and Dr. Viljoen indicated they would not be doing any of the work until advice from their lawyer was received. They also did not think that requesting MIFU for the centrifuge to ensure proper cleaning and disinfecting procedure is followed to be relevant in addressing the complaint that was bloodwork related.

- December 18, 2023 inspection:1. Chair for clients to sit and have blood drawn is ripped and repaired with tape. Dirt has accumulated along the tape. Two additional cracks were noted on the chair. Please replace chair cover to allow easy cleaning and disinfection after each client use. 2. Wiping cloth that is used with the Cavicide spray is changed once daily. If reusable wiping cloth is used, it must be laundered after each use to prevent re-contamination of the surfaces that's been cleaned and disinfected. Spray once and wipe to clean. Spray the second time and leave the surface wet for the required contact time to disinfect. Feb 28, 2024 reinspection note:Dr. Viljoen and Ramel indicated they would not be doing any of the work until advice from their lawyer was received

- December 18th, 2023 inspection:1. Paper towel is not easily accessible from the dispenser as the faucet was installed in the wrong direction. Please relocate the paper towel dispenser to make it accessible.2. Handwashing soap was not easily accessible as items for bloodwork were kept surrounding the hand washing sink. Please clear the hand sink counter to ensure hand soap is easily accessible for hand hygiene.3. Biomedical waste was almost filled to the top, past the line set for receptacle replacement. Biomedical waste receptacle should not be more than 3/4 full. Please replace accordingly.
