      • All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST RECAULK AROUND REAR MOP SINK AND REMOVE ALL MILK/POP CRATES BEING USED AS SHELVES.

      • All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. MUST REMOVE ICE BUILD-UP IN REAR ICE CREAM FREEZER AND CLEAN INTERIOR OF IT. MUST DETAIL CLEAN-REMOVE RUST FROM 2-COMPARTMENT SINK AND SHELVES IN REACH-IN COOLER/FREEZERS.

      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST DETAIL CLEAN FLOORS UNDER/AROUND HOT WATER TANK, MOP SINK AND ALL HEAVY EQUIPMENTS, IN PHARMACY, STORAGE AREAS AND THROUGHOUT. MUST REPLACE DAMAGED FLOOR TILES UNDER MOP SINK.
    • 35. WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS
      • The walls and ceilings shall be in good repair and easily cleaned. MUST CLEAN WALLS AROUND DUMPSTER AREA, SEAL AROUND ALL PIPES/CONDUITS, REPLACE DAMAGED WALL BY MOP SINK AND WALL BETWEEN TOILET ROOMS. MUST REPLACE MISSING TILE PIECES IN BOTH TOILET ROOMS AND REPLACE ALL MISSING RUBBER WALL BASE THROUGHOUT AS NEEDED. MUST ALSO REPLACE REAR EXIT DOOR AT THE BOTTOM WITH RUST SPOTS NEAR PHARMACY.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST PROVIDE WORKING VENTILATION TO THE TOILET ROOMS.
