      • Mouse droppings were observed in the following areas:- On the floor of the dry‑storage shelving in the kitchen- On the floor of the basement storage roomPlease clean up the droppings and disinfect all affected areas with 1000 ppm bleach solution and continue the following: - Engage a licensed pest control company to assess and address the issue. Submit their inspection and treatment reports to AHS for review.

      • The QUAT sanitizer concentration in the kitchen and 2nd‑floor servery sanitizer buckets was measured below 200 ppm. A fresh solution was dispensed from the dispenser, and the violation was corrected onsite.Please ensure QUAT sanitizer is maintained at 200 ppm at all times.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • 1. The paper towel dispensers at the kitchen hand sinks were found empty. Hand sinks must be stocked with appropriate hand‑washing supplies at all times.2. 2nd floor servery: A tray of cups was placed at the hand sink beneath the soap dispenser, obstructing access and making proper handwashing difficult. Hand sinks must remain unobstructed and accessible at all times. Please ensure nothing is stored in or around hand sinks.The violations were corrected onsite.

      • Mouse droppings were observed in the following areas:- On the lower shelf and floor of the dry‑storage shelving in the kitchen- On the shelves and floor of the basement storage room- In the corners around the fridge in the basement servery and underneath the steam tablePlease clean up the droppings and disinfect all affected areas with 1000 ppm bleach solution and continue the following: - Engage a licensed pest control company to assess and address the issue. Submit their inspection and treatment reports to AHS for review.
