      • Observation: (CORRECTED DURING INSPECTION) Foods stored in the cooler by the meal prep area were left uncovered and stacked on above each other. Public Health significance - Foods left uncovered are prone to cross contamination from other food items, airborne contaminants and contact surfaces which can lead to foodborne illness and spread of pathogens.
      • Corrective Action: Operator discarded food items not protected from contamination voluntarily and sanitized cooler. Ensure all food items stored are covered to prevent cross contamination from other food items and spread of pathogens.
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Food items (pieces of onions) littered on the floor of the walk-in cooler. Public health significance - Littered food items on the floor of the walk-in cooler could serve as attraction for pests, encourage the growth of pathogens and cross-contamination of other food items.
      • Corrective Action: Operator to ensure the walk-in cooler is cleaned at all times and free of food debris.
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Dirt and pieces of food items (shrimps) under the shelves of the walk-in freezer. Public health significancen - Accumulation of food debris can encourage the growth of pathogens and cross-contamination of food items.
      • Corrective Action: Operator to ensure the walk-in freezer is cleaned regularly.
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: (CORRECTED DURING INSPECTION) Food contact items (patty paper) stored together with garbage in the supplies storage room. Public health significance - Garbage may contaminate food contact surfaces and increase risk of illness.
      • Corrective Action: Operator discarded contaminated patty paper. Ensure garbage is not stored in the supplies storage room.
    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Mice droppings noted in basement storage area and electrical room; Public Health Significance - Pests are vector of diseases and pathogens, and they could cross-contaminate/contaminate food items & food contact surfaces (also frequently touched surfaces) with feces and urine which could lead to spread of diseases and pathogens.
      • Corrective Action: Operator to clean mice droppings in the affected area in a manner that prevents aerosalization of droppings (by avoiding sweeping and vacuuming). Operator to inform their pest management service of the situation and follow their recommandations.
    • 306 - Repeat Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: (CORRECTED DURING INSPECTION) General sanitaiton of kitchen was poor. Build up can be observed on floor and equipment. Public health significance: Unclean and unsanitary kitchen area can serve as a breading site for pathogens and attract pests.
      • Corrective Action: Operator to ensure kitchen area and food premises in general are clean and sanitary.
    • 402 - Critical Employee does not wash hands properly or at adequate frequency [s. 21(3)]
      • Observation: Food handler touched food contact surfaces, handled items such as spatula and spoon at the sandwich prep area and returned to another meal prep area without washing hands. Public health significance - Hand washing is the most effective intervention that helps prevent the food contamination, cross-contamination of food and food contact surfaces and spread of diseases and pathogens.
      • Corrective Action: Operator to ensure staff members are educated on the importance of hand washing.

      • Observation: Observed waffle irons to be covered with grease. (Public Health Significance) - Accumulation of grease on food handling equipment such as waffle irons can result in physical contamination of food. If such contaminated food items are consumed, they may result in adverse health effects.
      • Corrective Action: Operator to ensure that the waffle irons are thoroughly cleaned and sanitized on a frequent basis.
    • 306 - Repeat Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Observed accumulation of grease and grime on food contact surfaces such as shelving, storage counters, behind and on the sides food handling equipment such as the deep fryer. (Public Health Significance) - Accumulation of grease and grime on food contact surfaces and food handling areas can potentially cause contamination of food items by aerosolization.
      • Corrective Action: Operator to ensure that all the hard to reach areas of the food premises are throughly deep cleaned on a routine basis.

      • Observation: Pipe below the dishwashing sink appeared to be leaking. Significant accumulation of water was also observed near the dishwashing area. (Public Health Significance) - Pooling of water in a food premises may cause physical contamination of food items and food handling equipment. Such contaminated food items if consumed may result in adverse health effects.
      • Corrective Action: Operator to ensure that the dishwashing areas is frequently cleaned so as to avoid pooling of water.

    • 201 - Critical Food contaminated or unfit for human consumption [s. 13]
      • Observation: (CORRECTED DURING INSPECTION) Observed (milk and creamer cartoons) stored beyond their best before dates in the refrigeration unit. (Public Health Significance) - There may result in growth of illness causing pathogens in food items post their best before dates. Consumption of such contaminated food items may result in food-borne illness.
      • Corrective Action: Operator to ensure to periodically monitor the best before dates of food items. Operator voluntarily discarded the food items past their best before dates at the time of inspection.
    • 205 - Critical Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
      • Observation: (CORRECTED DURING INSPECTION) Observed dairy product (milk jug) and chopped fruits stored above 4°C (temperature more than 7°C) in the bottom cooler by the coffee station. Potentially hazardous food items such as dairy and chopped fruits when stored in the temperature danger zone (4°C - 60°C) for more than 2 hours can cause growth of illness causing pathogens, such food items if consumed can cause food-borne illness.
      • Corrective Action: Ensure to store potentially hazardous food items (dairy and chopped fruits) outside the temperature danger zone (4°C - 60°C) at all times. Operator was able to store potentially hazardous food items at appropriate temperature at the time of inspection.

      • Observation: Observed several food items in the prep coolers inadequately and improperly covered (Public Health Significance) - Inadequately and improperly covering food items in the prep cooler can cause contamination of the food items by various physical and biological agents, consumption of such contaminated food items may result in adverse health effects and food-borne illness.
      • Corrective Action: Operator to ensure that all food items in the prep coolers are adequately and appropriately covered.

      • Observation: Observed scoops submerged in coleslaw salad in the prep cooler by the fryer (Public Health Significance) - Food handling equipment such as scoops that are exposed to bare hand contact, can cross-contaminate the food items when they are submered in them. Consumption of such contaninated food items can cause food-borne illness.
      • Corrective Action: Operator to ensure that food handling equipment such as scoops are stored in a manner (handles sticking out) to prevent cross-contamination and potentially food-borne illness.
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Observed significant accumuation of grease on the waffle irons (Public Health Significance) - Accumulation of grease on food handling equipment such as a waffle iron may result in physical contamination of food. If such contaminated food items are consumed, they may result in adverse health effects.
      • Corrective Action: Operator to ensure that the waffle irons are frequently cleaned and sanitized.
    • 306 - Repeat Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Observed pooling of oil and food debris on equipment storage counters in the kitchen. Significant accumulation of oil and food debris was also noted behind the fryer. Also observed dust bunnies on the lights above the waffle making area (Public Health Significance) - Unsanitary conditions caused by accumulation of oil, food debris and dust bunnies in the food premises can cause physial contamination of food. Such contaminated food items if consumed may result in food-borne illness and adverse health effects.
      • Corrective Action: Operator to ensure that the food premises is routinely cleaned and maintained in a sanitary manner.
    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: Temperature of the cooler by coffee station was measured above 4°C (7°C and above), hence the cooler was unable to maintain the required cold storage temperature of 4°C or below. Storing potentially hazardous food items in the temperature danger zone (4°C- 60°C) can cause growth of pathogens and potentially food-borne illness.
      • Corrective Action: Operator to ensure that the prep cooler by the coffee station is repaired/fixed such that it adequately meets the required storage temperature at all times.

    • 101 - Repeat Plans/construction/alterations not in accordance with the Regulation [s. 3; s. 4]
      • Observation: Wall tiles and ceiling in the dishwasher area were broken. Public health significance - The broken wall tiles and ceiling may provide a hiding place for pests. Additionally, a hanging ceiling tile could potentially harm someone if it were to come off completely.
      • Corrective Action: Tiles and ceiling needs to be repaired or replaced.
    • 306 - Repeat Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: There is a noticeable accumulation of grime, debris, and grease throughout the cooking area, particularly behind the deep fryer, inside the under-counter fridges, waffle machine and beneath the grill. Public Health Significance: The presence of grease, grime, and debris in food handling areas may lead to physical and biological contamination. Consuming contaminated food may result in foodborne illnesses.
      • Corrective Action: The operator must conduct a thorough deep cleaning and sanitization of the facility immediately.
    • 308 - Repeat Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: Under counter fridge gaskets were in unsanitary and damaged condition. Public health significance: Dirty gaskets may lead to cross-contamination of food. Cold air may escape from the fridge, which may lead to potential temperature abuse of food items stored inside those units.
      • Corrective Action: Damaged gaskets need to be replaced or fixed, and cleaning must be performed on the dirty gaskets.

      • Observation: Walk-in freezer floor was broken.     • 101 - Plans/construction/alterations not in accordance with the Regulation [s. 3; s. 4]
      • Observation: Wall tiles and ceiling in the dishwasher area were broken. Public health significance - The broken wall tiles and ceiling may provide a hiding place for pests. Additionally, a hanging ceiling tile could potentially harm someone if it were to come off completely.
      • Corrective Action: Tiles and ceiling needs to be repaired or replaced.
    • 203 - Critical Food not cooled in an acceptable manner [s. 12(a)]
      • Observation: Potential hazardous food (PHF), such as mushrooms and onions, were cooked and stored inside the under-counter fridge. The temperature was measured at 45°C, which does not follow the cooling step of the food safety plan. Public health significance - Cooking and cooling of PHF as per the food safety plan ensures minimum risk to consumer.
      • Corrective Action: Written food safety plans procedures should be followed to ensure that a health hazard does not occur in the establishment.
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Over the counter chopping board placed on left prep cooler near service window and the meat slicer were found in unsanitary condition. Public health significance - the cuts and grooves present on the chopping board may cross contaminate the food, which could lead to foodborne illness. Bacteria may grow on the leftover meat pieces present on meat slicer and potentially may contaminate fresh cut meat pieces.
      • Corrective Action: The chopping board needs to be resurfaced or replaced. Meat slicer parts needs to be taken apart and thoroughly washed, rinsed, and sanitized after each shift.
    • 306 - Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Mould and grease build-up was noticed inside the undercounter fridge compartment, ice machine, waffle machine, and under the grill. Public health significance - Mold and grease could be transmitted to the food and may cause foodborne illness.
      • Corrective Action: Deep cleaning should be performed for the mentioned surfaces.
    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: Walk-in freezer floor was broken. Public health significance - Keeping the floor in good condition reduces the chances of trip hazards and it's easier to clean the floor.

    • 305 - Repeat Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
      • Observation: Back door has a gap that can allow mice and pests to enter. Mice can fit through gaps as small as a pencil tip. Food is also left open which attracts mice and pests.
      • Corrective Action: Operator mentioned that there is a plan to replace the back door and temporarily he will be getting the door fixed
    • 306 - Repeat Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: General sanitation of the kitchen including the roof and floor of the kitchen area, deep fryer area, dry storage area, observed to have some build-up of food and debris. Operator mentioned there is an issue with the HVAC system and is currently working on fixing it.
      • Corrective Action: Operator to perform deep cleaning of the facility floors and roof and follow up on fixing the HVAC system.
    • 311 - Repeat Premises not maintained as per approved plans [s. 6(1)(b)]
      • Observation: Missing tiles and accumulation of oil and food debris is observed under the fryer. The woodplank placed under the fryer is not acceptable.This is a repeated violation observed since June 2020. Accumulation of grease on the floor can lead to pest infestation.
      • Corrective Action: Operator to deep clean the fryer area and provide a probable date of completion for fixing this outstanding issue.

    • 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation: Meat slicer beside the front kitchen area was not cleaned and sanitized properly and it contained the meat residuals attached to it under the hard to clean surfaces. The meat residuals left on the slicer is exposed to room temperature which may lead to growth of pathogens and toxin formation and may cross contaminate the other food products when the slicer is reused.
      • Corrective Action: The operator was told to dissemble the meat slicer and clean and sanitize the equipment as per the manufacturer’s guidelines

      • Observation: High temperature dishwasher was unable to meet the temperature mark of 71°C. Temperature was measured for atleast 4 times and the machine was run for more than 10 cycles. Improper sanitization of the dishes may lead to increase in growth of pathogens and toxin production which may lead to food borne illnesses.
      • Corrective Action: Operator has contacted Ecolabs to get the dishwasher fixed and in the mean time operator has set up sanitizer tubs with QUATS sanitizer.
    • 311 - Repeat Premises not maintained as per approved plans [s. 6(1)(b)]
