    • 201 - Critical Repeat Food contaminated or unfit for human consumption [s. 13]
      • Observation: (CORRECTED DURING INSPECTION) Observed two bulk containers of Alfredo cream sauces contaminated with bacteria and mold at time of inspection . (Public Health significance) - All foods on the premises must be wholesome and free from spoilage or adulteration and otherwise fit for human consumption. Unacceptable food items could cause food borne illness and become insect and rodent attractants or inadvertently be added to other food. Operator discarded the contaminated food sauces at time of inspection.
      • Corrective Action: Operator is to ensure all food products within the walk-in refrigerator are fit for human consumption. At time of inspection two containers of Alfredo sauce were observed to have mold and bacteria on both the exterior container and within the container lid and opening. Bacteria and mold in food products can cause food borne illness, especially in vulnerable populations.
    • 209 - Repeat Food not protected from contamination [s. 12(a)]
      • Observation: Foods were left uncovered and open to contamination. Observed butterscotch pudding uncovered in the walk-in refigerator at time of inspection. (Public Health significance) - Contamination of food from contact with objects, people, pests or chemicals can cause serious illness.
      • Corrective Action: Cover or otherwise protect all foods from airborne contaminants, drips from raw foods, and contact with objects and staff
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Observed build-up of biofilm on the self-serve coffee machine within the cafeteria at time of inspection. Biofilm from bacteria accumulation in moist environments / conditions can directly contaminate beverages leading to potential foodborne illness.
      • Corrective Action: Operator to adequately remove all biofilm, clean and then sanitize the water dispenser and receiving tray on the equipment and ensure the self-serve coffee machine is maintained in sanitary condition.
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Observed build up of food debris and bacteria on the metal wire rack shelving within the walk-in refigerator at time of inspection. Food debris accumulation can be an attractant for potenial pests and permit continous bacterial growth within the walk-in refigerator. Food storage surfaces not maintained in sanitary condition can permit cross-contamination into food products leading to potential food borne illness.
      • Corrective Action: Operator to thoroughly remove all old food debris and bacteria from wire shelving, then clean and sanitize the wire shelving to prevent microbial growth on food storage surfaces that can cross-contaminate food products leading to potential food borne illness.
    • 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation: (CORRECTED DURING INSPECTION) No active sanitizer detected in sanitizer containers or spray bottles. (Public Health Significance) Sanitizing solutions must be strong enough to destroy bacteria on surfaces to prevent the contamination of staff, clients or food on contact with the improperly cleaned surface. Observed operator fill a dedicated food contact surface sanitizer container at time of inspection with a 200ppm QUAT solution.
      • Corrective Action: Change sanitizing solutions as required to always maintain an effective concentration (200 ppm unscented bleach, 200 ppm QUAT)- follow manufacturer's specifications)
    • 307 - Equipment/utensils/food contact surfaces are not of suitable design/material [s. 16; s. 19]
      • Observation: Observed cardboard used for the storage of produce, prepackaged cheese and prepackaged drink products within the walk-in refrigerator at time of inspection. Cardboard can retain moisture within the cold-holding equipment, permitting both the growth of bacteria which can directly contaminate food products and the adherence of bacteria onto cold-holding storage shelving surfaces, walls, fans and floors. These surfaces can cross-contaminate food products, which can result into potential food borne illness.
      • Corrective Action: Operator is to ensure all produce food products stored within the walk-in cooler are in food-grade, non-toxic containers with lids to prevent food being contaminated by dust, dirt, debris and people.

      • Observation: (CORRECTED DURING INSPECTION) Adulterated foods on premises. All foods on the premises must be wholesome and free from spoilage or adulteration and otherwise fit for human consumption. Unacceptable food items could cause food borne illness and become insect and rodent attractants or inadvertently be added to other food. Observed the operator voluntarily discard multiple Kraft brand condiments, dressings, sauce containers, pickles and packaged pear puree that were covered in mold and mildew from the walk-in refrigerator into the garbage.
      • Corrective Action: Remove all foods that are spoiled, adulterated or otherwise unfit for human consumption from the premises.
    • 209 - Repeat Food not protected from contamination [s. 12(a)]
      • Observation: Observed scoops left in large quick oat and wheat product ingredient bins. Improper scoop storage exposes items to bare hand contact and potential contamination.
      • Corrective Action: Operator shall store scoops so that foods are not exposed to the handles or bare hand contact. Operator to ensure scoops for food products are stored in a sanitary manner and are cleaned and sanitized regularly.

      • Observation: Observed various frozen vegetables within the walk-in freezer improperly stored which were left uncovered and open to potential sources of contamination. Contamination of food from contact with objects, people, pests or chemicals can cause serious illness.
      • Corrective Action: Cover or otherwise protect all foods from airborne contaminants, drips from raw foods, and contact with objects and staff

      • Observation: Observed various frozen food items stored on the floor of the walk-in freezer at time of inspection. Food items stored on the floor can become contaminated food from contact with objects, people, pests or chemicals, which can cause serious illness.
      • Corrective Action: Operator is to ensure that food products stored within the refrigeration equipment are adequately stored at a minimum of 6 inches (15 cm) from the floor, to prevent contaminants, drips from raw foods, potential pests, contact with objects and staff from contaminating food products and permit thorough cleaning of floor area within the cold-holding equipment.
    • 301 - Critical Repeat Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Observed multiple areas within the premise that were visually soiled with food debris, grease, and grime contaminants on food preparation surfaces, equipment, and utensils (e.g. stainless steel shelves where breakfast items and bowls are stored, along the counter where the self-dispensing coffee unit is stored and out-of service hot-holding serving station is situated, serving carts).
      • Corrective Action: Operator to ensure staff properly understand, are trained and follow all sanitation procedures within the premise to reduce the risk of pathogenic organisms harbouring and growing on food service equipment.
    • 304 - Repeat Premises not free of pests [s. 26(a)]
      • Observation: Evidence of mice; droppings were noted in the facility. Observed multiple droppings under the commercial automatic dishwasher and along the flooring underneath the three-compartment sink where a large opening in the wall was observed. Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.
      • Corrective Action: Remove any pest attractants such as food and water, eliminate access to the building and routes they use inside. Trap and remove the pests. The use of a professional pest control operator is recommended. Clean and sanitize area where droppings are located and monitor situation.
    • 305 - Repeat Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
      • Observation: Observed large unsealed gaps on the interior walls where conduit and plumbing service openings are present. Openings or gaps within walls can permit harbouring and breeding of pests within the premise and permit movement of pests from dish washing area to the food preparation, storage and food service area which can contaminate food contact surfaces, utensils, and food products. Observed various organic food debris in walk-in refrigerator floors, and dishwashing area.
      • Corrective Action: Operator to repair and / or seal all interior wall gaps around electrical conduit and plumbing service openings to inhibit and prevent pest movement, harbouring and breeding within the premise. Operator to thoroughly clean and remove all food debris observed on floors and counters to reduce pest food resource and presence.
    • 306 - Repeat Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Observed multiple areas within the walk-in refrigerator with mold, mildew, and food residue. Wire shelving, flooring, walls, serving carts, cardboard boxes and food items were all observed to be covered in mold, mildew or old food debris. Cross-contamination can permit microbial growth in food items resulting in potential foodborne illness.
      • Corrective Action: Operator to have a service technician or maintenance personnel repair water leak within the walk-in cooler and provide environmental health officer with a receipt of work completed. Operator is to remove all food items from the walk-in refrigerator, physically remove and discard mold, mildew and food debris from the wire shelving, floors and walls, clean and sanitize the refrigeration equipment and all wire shelving, serving carts, walls and floors and utilize food-grade containers for the storage of food items within the refrigerator.
    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: Observed the large walk-in refrigerators' flooring to be saturated with stagnant water, which was seen pooling under wire shelving. Observed walk-in refrigerator door unable to properly close and seal due to an issue with the latching mechanism on the equipment's door handle. A fridge door that won't close properly can lead to temperature fluctuations, frost build up, food spoilage and permit microbial growth on both food and food contact surfaces.
      • Corrective Action: Operator to have a service technician or maintenance personnel repair the walk-in refrigerator door, confirm there is a proper seal to ensure potentially hazardous food items are stored 4°C (40°F) or lower, and provide environmental health officer with a receipt of work completed

      • Observation: (CORRECTED DURING INSPECTION) Adulterated foods on premises. Unacceptable food items could cause food borne illness and become insect and rodent attractants or inadvertently be added to other food.       • Observation: Observed tubular florescent lights without plastic safety casings in dry storage area. Broken glass from the lighting source without safety covers / casings permit a direct physical hazard of glass contamination into food products.
      • Corrective Action: Operator to purchase, install and ensure that all tubular florescent lighting bulbs in the premise, and especially in food preparation areas are properly secured and covered with the appropriate safety barrier casings.

      • Observation: Observed multiple areas within the premise that were visually soiled with food debris, grease, and grime contaminants on food preparation surfaces, equipment, and utensils (e.g. stainless steel shelves where breakfast items and bowls are stored, along the counter where the self-dispensing coffee unit is stored and out-of service hot-holding serving station is situated, serving carts).
      • Corrective Action: Operator to ensure staff properly understand, are trained and follow all sanitation procedures within the premise to reduce the risk of pathogenic organisms harbouring and growing on food service equipment.
    • 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation: (CORRECTED DURING INSPECTION) Inadequate sanitizing rinse solution. No sanitizer detected at time of inspection. Observed large sanitizing solution container empty while automatic dishwasher was in use. Operator changed the empty sanitizing chemical bucket with a new and full sanitizing solution container and re-washed all utensils, pans, pots and food articles at time of inspection to ensure adequate sanitization of multi-use articles and utensils used in food preparation and processing. Automatic dish washers must provide a minimum concentration of 50 parts per million (ppm) as required to effectively sanitize dishware, 100 ppm recommended; Hand washing requires 200 ppm unscented bleach solution.
      • Corrective Action: Operator to monitor and log sanitizer concentrations of the automatic dishwasher regularly to know when the unit needs adjustment or repair or when sanitizing solutions should be changed.
    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Evidence of mice; droppings were noted in the facility. Observed multiple droppings under the commercial automatic dishwasher and along the flooring underneath the three-compartment sink where a large opening in the wall was observed. Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.
      • Corrective Action: Remove any pest attractants such as food and water, eliminate access to the building and routes they use inside. Operator to contact the professional pest control company to conduct an inspection and provide environmental health officer with receipt of work completed by the pest control company. Operator is to discard all rodent droppings appropiately and clean and sanitize area(s) where droppings are located and monitor situation.
