- Personal services notification (PSN) was completed during the inspection. If you move locations or add/remove services in the future, please notify this office.

- Expired sterile equipment was observed. Remove all sterile critical equipment throughout the facility that has expired.

- Bacteriostatic saline is preserved and not permitted to be used for injection services that require cosmetic products to be reconstituted with non-preserved saline solution. Discussed the requirement to follow manufacturer's instructions for cosmetic products used for injection services.

- Please install thermometers in the refrigerators used to store cosmetic products used for injection services. Develop a system to monitor refrigerator temperatures to ensure products are being stored at correct temperatures as per the manufacturer's instructions.
