- Bleach sanitizer was 800ppm, a concentration higher than the approved level of 100-200ppm. Corrected during inspection. - Chlorine test strips had expired in 2021. Action Required:Please have in place valid chemical test strips to accurately verify the strength of your approved sanitizers e.g. Bleach 100ppm. Email PHI proof of purchase or photo evidence of test strips onsite by December 29, 2025.

Action Required:Please have in place an effective pest control program and conduct pest monitoring activities on a monthly basis to prevent conditions that may promote harborage of pests. Also keep the three most recent months' pest control reports onsite, when in operation.
