- The measured concentration of the chlorine sanitizer pail was 0ppm. Requested operator refill and prepare a fresh pail of sanitizer solution. Retested concentration was acceptable (100ppm).

- The measured concentration of the low temperature chlorine dishwasher was 0ppm. The chlorine sanitizer pail was full and numerous cycles were run. Please ensure corrective actions are taken and dishwasher repaired and sanitizer concentration are acceptable (100ppm).

- Chemical test strips were not available onsite. Please have in place Chlorine or other applicable test strips to test the strength of your approved sanitizers (Bleach 100ppm. QUATs 200ppm, 12.5ppm -25ppm). Please email PHI photo evidence of valid chemical test strips.

- Electrical socket by the stainless-steel upright freezer in the storage room was missing a wall plate. Please install a wall plate to ensure safe use of the electrical socket and to facilitate safe and effective cleaning procedures. Correct by November 12th and email PHI photo or video evidence.One of the storage room fluorescent light covers was missing. Please install a fluorescent light cover or replace current lights with shatterproof ones to prevent contamination of food and food contact surfaces in case of light bulbs breaking. Correct by November 12th and email PHI photo or video evidence.
