- *Ongoing Violation*Mouse droppings were noted in areas including but not limited to besides prep cooler, behind the dough mixer, and underneath the shelving rack in the dry storage area. - Operator is to ensure to remove droppings using appropriate methods and thoroughly clean and sanitize all the areas.**January 7, 2026: Multiple dead mice were observed in tin trap located underneath the chest freezer. Rodent droppings were also observed on the shelf besides the freezer.- Make sure to remove dead mouse and droppings using appropriate methods and thoroughly clean the area.- Ensure that adequate pest control is taking place in the facility.

- No surface sanitizer was available for use at the facility. Operator prepared chlorine sanitizer (100 ppm) solution during inspection.- As discussed, ensure that sanitizer solution is prepared, tested, and maintained at adequate concentration (100 ppm).

- Unassembled pizza boxes were observed stored on the floor in the back kitchen.- Please ensure all food and food related items are stored in a safe and sanitary manner at least 15 cm (6in.) off the ground.

- Paper towels were not placed in the dispenser at the washroom hand sink. - Operator is to ensure all hand wash sinks are fully stocked at all times with soap, paper towels, hot and cold-running water supply.
