      • Single use paper receptacles are being re-used to store cleaned equipment. Bags are single use only and are not to be re-used. Cleaned equipment that were being stored in backs was recleaned and disinfected.

      • 1. Low Level disinfectant (RTU) was being used to top up high level disinfectant (CS20). 2. Containers of disinfectant are being topped up, instead of emptied, cleaned and refilled.3. Physical residual was observed in disinfection chambers.

      • 1. Unnecessary articles (empty chemical bottles) are stored in an insanitary manner. Must remove all unnecessary articles from reprocessing area to prevent potential harborage of pest and to facilitate pest control management.2. Handwash station countertop is in disrepair. Replaced countertop with cleanable and non-absorbent surface.
