- The hand sinks in the nail service area, facials rooms, and washroom did not have paper towels or any means to dry hands.Obtain paper towel supplied in a dispenser.

- The Hydro Dermabrasion Machine was visibly dirty. The suction head had debris and residue accumulated in it.Thoroughly clean and disinfect the hand pieces that comes into contact with the clients skin. This must be done after each service is preformed. Visually inspect the condition of the hand pieces prior to conducting the service.

- Debris accumulation on tweezers, wooden spatulas, threads, and in containers for storing waxing supplies. Adequately clean and disinfect tweezers after use. Clean storage containers regularly. Store clean tweezers adequately separated from other supplies.

- 1. Reprocessed nail clippers, cuticle pushers, and metal callus shavers were visibly dirty. Ensure that these items are thoroughly cleaned with a proper cleaning tool (brush), to remove any debris prior to disinfection. Visually inspect equipment as they are cleaned.2. The pedicure basin contains a jet system that the operator had not opened or assessed for cleanliness prior to disinfection. Hair and lint were found entangled in the jet cover.The operator must be able to remove the jet cover to allow for thorough cleaning and disinfection of the pedicure basin. All components must be cleaned between clients.

Ensure that surfaces and equipment are disinfected with an approved disinfectant with a DIN or NPN. Ensure that disinfectants are only used within expiration.3. Reprocessed bowls used for mixing facial products, as well as application brushes, were found with visible product residue. Additionally, the metal bowls were in poor condition, with the surface film visibly peeling.Ensure all reusable bowls and brushes are thoroughly cleaned and free of residue before reuse. Replace any equipment that is damaged or deteriorating.

- The piercing earrings were not sterile at the point of use. Additionally, the equipment lacked any documentation verifying sterility. The earrings were also stored haphazardly in an unsanitary container.Immediately cease the use of this piercing equipment. Only use piercing earrings that include clear sterility information on the packaging, as provided by the manufacturer.

- 1. Facial cleansing devices were observed with visibly dirty bristle tips and product residue at the point of foam dispensing. The operator stated that these containers are provided to clients for direct facial or hand application using the bristle tip. The tips are reused between clients without any reprocessing or sanitization.Immediately discontinue the reuse of facial cleansing devices without proper reprocessing. All reusable components that come into direct contact with clients must be thoroughly cleaned and disinfected between uses. Alternatively, dispense the product into a separate container for individual client use or replace with single-use alternatives.2. The wax pot stationed at the hair dressing chairs had an accumulation on hair inside of it.Discard the wax pot. Ensure that the wax pot is protected from contamination.

- The facility was visibly dirty in almost every area of the facility, including floors, countertops, client chairs, and work surfaces in the hair and esthetic areas.Initiate a thorough clean-up of the entire facility.
