    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • The operator did not dilute the Barbicide according to the manufacturer's instructions. The operator was informed to follow the manufacturer's guidelines for proper dilution.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • A cloth hand towel was hanging in the washroom, used by both staff and employees. There was no single-use method of hand drying available. The operator was informed to provide a single-use method, and a label was placed on a basket indicating 'Clean Towels' for client clarification. The hand towel was removed at the request of the PHI.
    • 20. Is reusable equipment disinfected properly?
      • Non-critical items, including brushes, combs, and scissors, were soaked in Barbicide; however, the disinfectant solution had not been diluted in accordance with the manufacturer's instructions, thus disinfection could not be confirmed. These items were placed into properly diluted Barbicide at the request of the PHI.

    • 20. Is reusable equipment disinfected properly?
      • Some tools were not fully submerged in the barbicide solution for proper disinfection. The barbicide solution was topped up in each container to allow for full submersion and disinfection.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Some hair clipping build up was observed on lids and containers of hair products located by the work stations. The areas were cleaned upon request.

    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Completion of a personal services notification was not noted on file. - Please complete and submit a personal services notification at the following website:https://ephisahs.microsoftcrmportals.com/create-case-psn/
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures were not available at the time of inspection for cleansing of skin or accidental skin cut or puncture. - Ensure the above written procedures are completed and easily accessible onsite. See attached templates.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Hair was noted on clippers and scissors stored in drawers at multiple stations. - Ensure proper cleaning of equipment is completed with tools followed by visual inspection to confirm cleanliness prior to disinfecting equipment.
    • 20. Is reusable equipment disinfected properly?
      • Multiple containers of Barbicide solution appeared cloudy with visual debris present. - Ensure to follow disinfectant instructions for use including changing daily and whenever solution is contaminated. Operator emptied and remade all cloudy Barbicide solutions.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Unlabelled spray bottles noted at various stations throughout the facility. Operator stated spray bottles contain water. - Label all spray bottles for proper identification of contents and prevention of accidental chemical hazard exposure.
