   • Did operator provide notice of intention, operation, additional services, and construction?
     • Operator failed to provide written notice to the Grand Erie Public Health of the intention to operate a personal service setting at least 14 days before starting the operation
     • The written notice shall include the name and location of the business, operator name and contact information and list of personal services provided.
   • Disinfectants: Did operator follow manufacturer instructions?
     • Do not use expired disinfectants
   • Did operator provide an explanation of the procedure, information about risks associated with the procedure and maintain records for invasive procedures?
     • Keep records for invasive procedures including, which procedure was done and the part of the body the procedure was done to, client name and contact information, service provider name and contact information, explanation of and risks associated with the procedure, date of procedure and lot numbers and expiry date of the pre-packaged sterile equipment used in the procedure.
     • Keep invasive procedure records for one year at personal service setting and keep readily available for an additional two years.
     • Provide the records to a medical officer of health or public health inspector on request.
