    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • *** Written procedures for skin cleansing procedures were not available.Corrective Actions: Complete page 2 (cleansing procedure) of the AHS written procedure template that was provided during the inspection. Ensure these procedures are either accessible or visible to all workers in the facility.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Pedicure and manicure equipment were kept in a clean and sanitary condition.*** Visibly dirty scissors, tweezers and single use lash curlers were found in the waxing room. These items were contaminated with hair and wax.Corrective Actions:- These items were removed. Going forward, ensure all equipment used during a services, including tweezers used during a waxing service, are taken to the reprocessing area to be cleaned and disinfected before they are returned to the waxing room.- Ensure cleaned equipment are stored in clean containers.
    • 20. Is reusable equipment disinfected properly?
      • Preempt CS20 disinfectant and AHP 7 test strips were available.*** Disinfectant was not tested daily, and results were not documented as required.Corrective Actions:- Test the disinfectant each day it is used.- Document the results of each test in a logsheet. AHS HLD logsheet was provided during the inspection. Recommend that both copies are laminated, and reused, ensuring that there will be one compete set of records are available for review.

    • 20. Is reusable equipment disinfected properly?
      • Daily high level disinfectant test results are not being recorded on the high level disinfectant log. -Ensure the high level disinfectant is tested daily with test strips and that results are recorded on the high level disinfectant log. The operator tested the high level disinfectant with corresponding test strips and recorded a "pass" result correctly on the high level disinfectant log sheet.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • One small open container of cuticle oil was stored in a manicure station drawer with a nail brush in the oil. - Ensure contaminated equipment is not stored within the bulk supply and that the bulk supply remains covered to protect from contamination. The operator removed the cuticle oil from use and the nail brush was removed to be cleaned. Dispensing smaller amounts of cuticle oil in smaller containers that can be used for each individual client is recommended if a brush is dipped into the oil.

      • Written procedures were not available for review at the time of inspection.- Ensure the required written procedures are completed and available for review onsite including the process for cleansing skin prior to waxing services and the procedure to follow in the event of an accidental skin cut or puncture.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Used uncleanable nail files were stored in the drawers of various pedicure stations. - Ensure uncleanable items are discarded after one use. The operator discarded the used nail files.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Nail brushes with visible debris were stored with clean equipment at two different manicure stations. - Ensure all used equipment is separated from clean equipment for reprocessing. The operator removed the nail brushes to be reprocessed.
    • 16. Is reusable equipment handled and stored properly (Non-Critical)?
      • Linen on the waxing bed noted to be soiled with visible debris. - Ensure linens are changed in between clients if they directly contact skin, body, nails, teeth, hair, if the service involves puncturing the skin or mucous membranes, or if they become visibly contaminated. The operator removed the linens from the waxing bed.
    • 20. Is reusable equipment disinfected properly?
      • Used semi-critical equipment was reported to be cleaned and then placed in a UV sterilizer. No high level disinfectant, high level disinfectant log or test strips were noted onsite at the time of inspection. - Ensure all semi-critical reusable equipment is cleaned and then disinfected with, at minimum, a high level disinfectant. The operator obtained an appropriate high level disinfectant, test strips, and record log during the inspection and began disinfecting cleaned semi-critical equipment as required.
    • 24. Are waste sharps disposed of properly?
      • A sharps container was not available during the time of inspection. - Ensure an appropriate sharps container is available for credo blades.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Brushes were stored in open containers of cuticle oil within the drawers of the manicure stations. - Ensure contaminated equipment is not stored within the bulk supply and that the bulk supply remains covered to protect from contamination.

      • Open, uncovered packages of eyelash extensions were stored in a manner that did not protect them from contamination on top of a table with visible glue and debris.- Ensure all cosmetic products are stored in a sanitary manner that protects them from contamination.

      • Various nail polishes displayed did not have proper labelling or available information such as a list of ingredients or manufacturer's instructions for use. - Ensure all cosmetic products are labelled properly with available information such as ingredients and manufacturer's instructions for use. The operator removed the nail polishes for personal use.Eyelash and eyebrow dye with no labelled manufacturer or available manufacturer's instructions for use was noted at the eyelash services station. - Ensure all cosmetic products are labelled properly including manufacturer information and that manufacturer's instructions for safe use are available. The operator discarded the dye.

      • Hydrogel eyepatches stored in the eyelash service station were noted to be expired on 01Aug2024. - Ensure the manufacturer's instructions for use are followed including when the product is considered expired. The operator removed the eyepatches for personal use.

      • Bottles containing cosmetic products located at the pedicure and manicure stations were not labelled and were reported to be portioned from a bulk supply as per the operator. - Ensure cosmetic products portioned from a bulk supply are labelled with the product identity.
