- Written procedures were completed during the inspection. Written procedures for cleansing skin were completed for the following services: -manicure -pedicure -waxing -eyelash extensionWritten procedure for accidental skin cuts or punctures was provided to the operator at the time of inspection.Reprocessing sink was not used for handwashing therefore written procedures for sink decontamination were not required.

- 1. Used nail files were discarded at the time of inspection upon request. Discussed requirement to discard single-use items after each use. The following tools are considered to be single-use: foam/cardboard nail files, emery files, buffing blocks, toe separators, cardboard covers for nail drill bits etc.2. Battery operated tool used to curl eyelashes was discarded at the time of inspection because it was uncleanable.

- 1. The high-level disinfectant (HLD) tray was too small. The cheese-grater style foot files were not being properly disinfected. Obtain a plastic/glass tray with a lid that is large enough to fully soak the cheese-grater style foot files.2.High-level disinfectant logs were not being maintained. Templates were provided to the operator at the time of inspection.3. There were multiple high-level disinfectant trays. High-level disinfectant logs were not being maintained for each high-level disinfectant tray. Recommended only one high-level disinfectant tray.

- Eyelashes were allegedly being cleansed using diluted baby shampoo but the original/bulk supply was not on-site at the time of inspection. Requested operator obtain a new cleanser and properly label the decanted foaming dispenser with its contents.
