- Written procedures are not available at the facility.AHS written procedures were discussed with the operator and would be sent with this report. Ensure written procedures are available on site.

- There is no antiseptic for skin preparation.Ensure antiseptic product with a DIN is obtained and used during skin prep.

- Sterility information is not been recorded with client records.Ensure sterility information for pre-purchased sterile equipment is recorded with each client record.

- Low level disinfectant is being used to disinfect semi-critical tools.Ensure semi-critical tools are cleaned, dried and immersed in high level disinfectant for the manufacturer indicated contact time. Used disinfectant may be discarded after every use, OR used for 5-7 days with daily confirmation of potency using a test strip and maintain daily test records.
