      • Some hairbrushes stored in a clean drawer beside the reprocessing area were observed with hair on them.- Please remove, clean the brushes and repeat reprocessing/disinfection.Ensure to properly clean and visually inspect brushes and other equipment before disinfection to prevent cross-contamination.
    • 20. Is reusable equipment disinfected properly?
      • A bottle of PREempt HLD5 in the Esthetics room was expired. No unexpired bottle was available onsite to replace.- Please discard and obtain an unexpired one.Ensure that only unexpired products are in use according to manufacturer's instructions.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1. The reprocessing sink and surrounding area were observed with visible dirt, debris, and stains.Please thoroughly clean.Ensure the reprocessing area is routinely cleaned and maintained in sanitary condition to prevent potential contamination of disinfected instruments. 2. Linen placed on treatment bed in the laser room was observed with visible hair on it.- Please remove and clean the linen.Ensure that all linens used for treatments are clean, free of visible debris/hair, and protected from contamination prior to client use.

    • 20. Is reusable equipment disinfected properly?
      • 1) The HLD5 and the CS20 disinfectant required for the disinfection of semi-critical reusable tools used in providing eyelash extensions were expired in December of 2023 and March 2023 respectively. 2) Not all of parts of the semi-critical tools are being adequately submerged in the disinfectant solution and therefore inadequately disinfected. - Wash the tools with detergent and water, rinse, dry and fully submerged in the disinfectant for the required contact time provided by the manufacturer. Rinse after disinfection. - Disinfectant must be discarded daily or concentration verified daily with the appropriate test strips as per manufacturer's instructions. - Do not provide eyelash services until the concerns above have been addressed and disinfectant has been obtained.
