      • Written procedures were not available on-site for review and were not provided via email when requested.Ensure written procedures are developed, maintained, and readily available on-site for staff reference and inspection. Procedures must include:1. Management of accidental cuts or punctures.2. Skin preparation prior to providing services.3. Aftercare instructions provided to clients.4. Proper use and maintenance of the dual-purpose sink used for both handwashing and tool cleaning.

      • No paper towels were available at the handwashing station in the waxing room. The operator was advised to ensure paper towels are provided at all handwashing stations at all times.

      • Repeated Violation: Client Record Documentation2026-07-06: During the inspection, it was noted that sterility indicators and lot numbers for piercing devices are still not being recorded with client records. Operator was advised that client records must include sterility indicator results and the lot number of the piercing device used.2025-02-28: Sterility indicator and lot number of piercing device are not being kept with client records.Lot numbers are not available with the type of piercing cartridges being used.

      • Hair/debris was observed on scissors stored in a container designated for clean instruments. The operator was advised to ensure instruments are properly cleaned and free of contamination prior to storage with clean items.

      • Pre-empt wipes were observed to be expired (expiration date: 03/2026). The operator was advised to replace the expired wipes with valid one.

      • The single sink in the waxing and piercing room is being used for handwashing and reprocessing.Ensure a written procedure is posted above the sink to indicate sink disinfection between different tasks.April 05, 2024: The required written procedure for dual purpose sink disinfection in-between use (handwashing and reprocessing) was still not observed. Please create the required written procedure. A template will be sent with this report.
