- Facility did not notify that they were adding services (Facials). Please note, going forward all new services including services done during "special events" that are outside the regular service offerings require notification to PHI.

- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Facility has a Doctor come in to do fillers and Botox. Client records were not available for view as they are kept with the service provider.Please keep a copy of the client's records onsite for at least 2 years. Records must also include sterility assurance documentation.
- 20. Is reusable equipment disinfected properly?
- 1) Disinfectant product meant to be used make-up brushes did not have a DIN. Please purchase and use a disinfectant with a DIN or NPN.2) There was no disinfectant observed for use on electric hair clippers (e.g. Clippercide or Wahl disinfectant). Please purchase an appropriate disinfectant and ensure stylists are using it to clean the appropriate tools.
