- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?

- Facility specific written procedures have not been completed. Templates for procedures were forwarded to the operator just prior to the inspection. The dual use sink procedure is required if the sink in the treatment room will be used as a hand sink.
- 20. Is reusable equipment disinfected properly?
- The operator utilizes CS 20, a high level disinfectant to disinfect tools used around the eye. No test strips or record of disinfectant strength is available. Test strips for CS 20 shall be obtained and used to test the CS 20 on each day it is used. The results shall be recorded. A record sheet will be forwarded with this email.
