      • 1. Some tools like tweezers were being disinfected in barbicide, this is inappropriate since they are semi-critical tools that should be disinfected in a high-level disinfectant. These items must be disinfected in high-level disinfectant. Please re-iterate this requirement with staff.2. The high-level disinfectant tray should have a high-level disinfectant that can be monitored. Ensure tools are being fully soaked as required.3. High-level disinfectant log was not available during inspection. Re-iterated the requirements on how to properly maintain HLD logs. 4. The AHP7 indicator test strips was also not available. Obtain new AHP7 indicator test strips.
