    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Staff were unaware of where the written procedures were located. Written procedures for skin cleansing and accidental injuries must be available.     • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • A skin cleanser was not available onsite for cleansing the skin prior to waxing. Purchase a skin cleanser (Ex. cleanser, makeup wipe)
    • 11. Is post-service care provided by personal service workers following any personal service or accident where a client's skin or mucous membrane is cut or punctured during a personal service, or when energy-emitting equipment is used?
      • When staff were questioned about their accidental injury procedures, they were unaware of the protocols. Staff were able to locate Infalab Liquid Skin Protector onsite after some searching.     • 15. Is reusable equipment handled and stored properly (Critical)?
      • Esthetic tools like nippers and clippers were being stored on the drawer handles at the manicure stations. Store esthetic tools in a clean and sanitary location.
    • 20. Is reusable equipment disinfected properly?

    • 20. Is reusable equipment disinfected properly?
      • Upon reviewing the HLD daily testing log, it appears that the HLD is being used beyond the 14-reuse period, in contravention of the manufacturer's instructions for use.Ensure that the HLD solution is discarded at the end of day 14 and start a new pour with a new HLD testing log on day 15. In the event that the HLD test fails before day 14 of the reuse period, the HLD solution must be discarded, and a new pour will be required.

      • The procedure previously established were not available in the facility for review. AHS template for facility specific procedures were provided onsite for client care, skin cleansing and decontamination of dual-purpose sink.Complete the procedures and send pictures via email to PHI.

      • No testing log sheets were maintained
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • The spray bottles of Lysol contained disinfectant ES15. Label the bottles indicating their contents correctly to allow safe use of their contents.

      • Reprocessing sink was being used for hand washing.Use washroom sink for hand washing. Put label on the reprocessing sink "NO HAND WASHING"

No test strips were available.3-No testing log sheets were maintained Follow the directions on the label of the disinfectant for required contact time, concentration and suitability for the equipment.
