- Violation Description:The facility does not have a written procedure outlining the steps to be taken in the event of an accidental skin cut or puncture during service.Corrective Action:- Develop, implement, and maintain a written procedure for responding to accidental skin cuts or punctures.- An AHS written procedure template was provided for this purpose. Also advised the owner that a Vietnamese version is available and will be provided via email.- Ensure all staff are made aware of this procedure.

- 1) Facility does not have test strips for high-level disinfectant. Corrective Action:Ensure compatible test strips are available and used to test the high-level disinfectant.2) Facility does not have a log to record the usage/testing of the high-level disinfectant.Corrective Action:Implement and maintain a log to record high-level disinfectant usage and testing results.Note: Advised facility to discard and refill high-level disinfectant daily until test strips are obtained.

- Staff indicated that a "Lysol" cleaner/disinfectant is used with a single application for both cleaning and disinfection, and that surfaces are kept wet for only a few minutes. Review of the product label indicated a contact time of 10 minutes is required for disinfection.Corrective Action:- Ensure the pedicure foot bath basin is cleaned and disinfected in accordance with the product's manufacturer's instructions (i.e. 10 minutes for the "Lysol" disinfectant product currently being used). - Cleaning shall be completed prior to disinfection, and surfaces shall remain visibly wet for the full contact time specified on the product label.- Provided recommendations on other disinfectant products that have a shorter contact time (i.e. 3-5 minutes).

- Staff were unaware of where the written procedures were located. Written procedures for skin cleansing and accidental injuries must be available. - A skin cleanser was not available onsite for cleansing the skin prior to waxing. Purchase a skin cleanser (Ex. cleanser, makeup wipe)

- When staff were questioned about their accidental injury procedures, they were unaware of the protocols. Staff were able to locate Infalab Liquid Skin Protector onsite after some searching. - Esthetic tools like nippers and clippers were being stored on the drawer handles at the manicure stations. Store esthetic tools in a clean and sanitary location.

- Upon reviewing the HLD daily testing log, it appears that the HLD is being used beyond the 14-reuse period, in contravention of the manufacturer's instructions for use.Ensure that the HLD solution is discarded at the end of day 14 and start a new pour with a new HLD testing log on day 15. In the event that the HLD test fails before day 14 of the reuse period, the HLD solution must be discarded, and a new pour will be required.
