      • Written procedures were not available for review. A template for written procedures was provided to operator via email.
    • 20. Is reusable equipment disinfected properly?
      • CS20 disinfectant was observed to be expired (September 5, 2024; Lot #090523VX06).The operator was instructed to obtain a valid disinfectant and reprocess all reusable semi-critical instruments. The operator was also instructed to cancel any eyelash extension appointments and discontinue providing eyelash extension services until all reusable instruments have been properly reprocessed using a valid CS20 solution.
    • 20. Is reusable equipment disinfected properly?
      • Accelerated hydrogen peroxide test strips were observed to be expired (expiration dates: 2019 and 2020). The operator was advised to obtain valid test strips and follow the manufacturer's instructions if they choose to reuse CS20 for up to 14 days. A high-level disinfectant testing log was provided to the operator via email.
    • 20. Is reusable equipment disinfected properly?
      • CS20 solution appeared old. Visible residue/sediment was observed at the bottom of the container. The operator was advised to discard the solution and follow the manufacturer's instructions for use and reuse.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • Same sink is used for handwashing and disinfection. Ensure the sink is cleaned and disinfected between handwashing and disinfection. A written procedure for the disinfection process is developed and posted at the sink.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • The operator stated that the spray tanning room floor was cleaned between clients using CLR (Calcium, Lime and Rust Remover) and Pine-Sol cleaner. The operator was advised that CLR and Pine-Sol are cleaning products and do not provide disinfection. The floor is required to be cleaned and disinfected between clients using, at a minimum, a low-level disinfectant.

      • - Test strips for the high level disinfectant was not observed. Required monitoring records were not available.Ensure test strips for the high level disinfectant CS20 is obtained, test and maintain daily records, OR discard used disinfectant after each use.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Required tanning signages were inadequate. Requirements and signage website were discussed with staff.Ensure signages are printed and posted at the required locations; customer entrance doors, facing customer at point of sale, facing employees at point of sale and near the artificial tanning equipment.
