- Disinfectant "Saber", DIN: 02362562, was being improperly diluted. Manufacturer's instructions require a 1:16 ratio of concentrate to water. This was calculated with Operator Brooklyn and corrected to ensure all spray bottles had the appropriate concentration. Disinfectant had a 5-minute wet contact time, per manufacturer's instructions. Operator was advised of this and to ensure this time is met.

- 20. Is reusable equipment disinfected properly?
- A high-level disinfectant could not be found in the lash room, the back staff room, or storage areas. A high-level disinfectant is required for all semi-critical items used, i.e., tweezers for eyelash extension application. Items must be first cleaned to remove any visible debris then the high-level disinfectant used following manufacturer's instructions. High-level disinfectant must be new for each use OR tested daily and re-used for a period of up to 14 days with a log (provided in accompanying email).
