    • 20. Is reusable equipment disinfected properly?
      • 1. High-level disinfectant, preempt CS20, was expired since April 2024.-Do not use expired product, please procure and use an effective high-level disinfectant to ensure proper cleaning and disinfection of equipment and tools.2. High-level disinfectant was used for more than one day without its concentration being tested with appropriate test strips and test results documented.-Please procure suitable test strip to monitor the concentration of high-level disinfectant and document the test results as required.3. Dirty lash scissor and tweezers were observed onsite during the inspection.-Please clean and disinfect lash scissors and tweezers with low-level disinfectant such as Mr. Clean or Lysol after each use.

      • Unsuitable sharp container was observed onsite.-Please procure appropriate sharp container and ensure proper disposal of sharps.Please take the filled containers of sharps to pharmacy shop around you for proper disposal of contaminated sharps.

    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • High-level disinfectant, preempt CS20 cleaning procedure was not available onsite.-Please paste the step-by-step cleaning procedure for CS20 at the reprocessing sink.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Porous and uncleanable equipment such as nail files and bluffers were not being discarded after use, used nail files and bluffers were seen onsite during the inspection.-Please do not reuse equipment such as nail files and bluffers to prevent spread of infection.
    • 20. Is reusable equipment disinfected properly?
      • 1. Alcohol was being used disinfect reusable semi-critical tools such as nail cuticles and scissors.-Please procure and use preempt CS20 or HLD5 to ensure proper disinfection.2. Pedicure basin disinfectant, preempt RTU, had expired since July 2021.-Please procure and use new preempt RTU to ensure proper disinfection of the pedicure basin.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Some of the cosmetic containers/bottles were not labeled with product name.-Please label all cosmetic containers with product name to prevent misuse.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Client service areas were observed with layers of dust and dirt.-Please clean and sanitize all client service areas immediately after use.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Alcohol was being used to clean/sanitize the client service areas.-Please use low-level disinfection such as preempt RTU, Lysol or others to sanitize all service areas after use.
