    • 20. Is reusable equipment disinfected properly?
      • * Preempt CS20 was not used according to the manufacturer's instruction.* Operator claims the solution of Preempt CS20 was changed daily. However, AHP 7 test strips revealed that the solution was ineffective (indicator strips were not changing colour, even well after 35 seconds). PHI tested the bottle of CS20 with the test strips and the results were a pass.* HLD test log was filled past the 14 day period.Corrective Actions taken:- Disinfectant tray was replaced with new disinfectant tested, and results recorded in a new test log.- Going forward, ensure the disinfectant is tested daily, and results documented in the test log.- Follow the manufacturer's instructions for contact time and reuse period (maximum 14 days).
