    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Brow tinting was not previously reported to AHS. Service has been added and PSN updated. Please ensure all new services are reported to the AHS.
    • 20. Is reusable equipment disinfected properly?
      • Records were not being kept for testing of the high-level disinfectant. CS20 was not being changed after the 14 day reuse period determined by the manufacturer. Disinfectant was changed and records completed for the day. Ensure the records are taken every day that the disinfectant is in use and dates for pouring the disinfectant are recorded so all staff know when to change the solution. Failure to test and change solution as needed can result in improper disinfection of tools that can lead to infection of clients.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Peel and stick shelf liner has been applied to the countertop around the reprocessing sink and is peeling away in several areas which will allow bateria to be trapped.Please remove the peel and stick liner and ensure surfaces are smooth, non-absorbent and easily cleanable.

    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Several used files and buffers were found in the manicure stations. These were discarded during the inspection. Single use items including files, buffers, toe separators, ect cannot be reused on a different client and must be discarded after use.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Tools that operator indicated were clean were found being stored in an unclean container. Nippers were also being stored in containers with brushes and other tools. All tools were taken for reprocessing. Store cleaned and disinfected tools in clean containers, drawers or covered. clean tools should be brought out at the beginning of a new client service and not left on workstations between clients.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Staff was observed placing tools directly into high-level disinfectant without cleaning, rinsing or drying first. Tools must be cleaned in soapy water, rinsed and dried before being placed in disinfectant. Soap and water loosen and remove debris, rinsing is necessary to remove the soap residue and drying is required to prevent diluting the disinfectant.
    • 20. Is reusable equipment disinfected properly?
      • 1. Tools in the CS20 container were not fully submerged, some were upside down with the clipper end not in solution at all. Solution was not full to the top of container to submerge tools properly. 2. No records have been recorded since July 10, 2025. 3. no test strips were readily available and those that were found were expired in 2024.Please ensure tools are fully submerged and container is filled completely. Please obtain test strips and ensure the high-level disinfectant is tested everyday the solution is in use and the results are documented.

      • Records were partially complete, but testing pass/fail was not recorded. Operator completed the days test results during inspection.
    • 24. Are waste sharps disposed of properly?
      • Waste sharps are being stored in a sharps container but operator indicated the container is then emptied into the garbage when full. Sharps must be disposed of in a safe manner in a container that protects people from potential accident exposures/punctures. Operator advised to return sharps container to the drugstore on the containers label.

    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Used toe separators were found in the drawers next to the foot baths. Staff indicated they are reused. These were discarded. Single use items must only be used once and then discarded. Do not store used items.
    • 20. Is reusable equipment disinfected properly?
      • 1. No test strips for the high-level disinfectant could be located. 2. No high-level disinfection records were being taken or recorded. Those posted were from March 2023.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1. The wax pot was left in an unsanitary manner.2. Drawers at the manicure tables were unclean and had dust in them.3. Tray holding waxing strips had dust accumulation in them. 4. waxing bed was left with used paper covered with dirt from the last client. Ensure areas are cleaned daily or as often as required. Single use surfaces should be discarded after use and areas cleaned after clients.

      • A sharps blade was left sitting on top of the sharps container. Sharps must be disposed of in the container to prevent accident exposures.

      • A scoop was left in the paraffin which could cause contamination of the product. Use a scoop with a handle and store it out of the wax in a sanitary manner.
