    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures needed for waxing, ear piercing, manicure and pedicure tools.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • No consent forms were used for Ear Piercing. All invasive procedures required consent forms and keep them for 2 years,
    • 20. Is reusable equipment disinfected properly?
      • 1) Hair clippers are not being adequately disinfected. Oster Blade Wash was used; it only cleans clippers and does not have a DIN. Ensure hair clippers are disinfected with a product intended to disinfect the clippers after cleaning.2) Pedicure and manicure tools were disinfected with low level disinfectant, PREEMPT RTU. Semi-critical tools require high level disinfectant such as PREEMPT CS20.

    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Antiseptic wipes were expired since 2024.Provide new antiseptic wipes.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • No consent forms were used for Ear Piercing. All invasive procedures required consent forms and keep them for 2 years,
