- A used lash brush was found in the disinfectant tray.The lash brush was discarded. Do not reuse single use items.

- 1) There was hair in the disinfectant tray with reuseable instruments. The disinfectant tray was cleaned and all reusable instruments in the disinfectant tray were cleaned and disinfected during the inspection. 2) There was hair on the hairbrush and silicone facial spatula that were stored in the cart. Clean and disinfect reusable instruments after use. Separate clean and dirty reusable instruments.Protect instruments and supplies from contamination during storage.

After the disinfection process, rinse instruments with water then dry.2) There was hair in the barberide in the men's section.The contaminated barbercide was discarded. Fresh barbercide was prepared during the inspection.

- The cushioned pad on the massage table was unclean. The material was absorbent and not easy to clean.Remove the cushioned pad from the client service area.

- The reprocessing area did not have sufficient space to separate dirty and clean equipment or to clean and disinfect equipment. Provide adequate space in the reprocessing area and maintain a one-way flow from dirty to clean.

- Alberta Health Services (AHS) was not notified of the new personal services added such as manicure and pedicure. Notify AHS prior to offering new personal services.
- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- 1) There were no written procedures for decontamination of sink that is used for both handwashing and equipment reprocessing.Complete written procedures. 2) The written procedures for skin cleansing only included facial.Update written procedures to include other personal services.

- This inspection revealed numerous of violations. It was evident that operators demonstrated a lack of knowledge of the Personal Services Regulation and Standards.

- Disposable gloves were found on the counter at the hair station.Used disposable gloves were discarded. Discard gloves immediately after use.

- Water was being used on clients' skin prior to waxing.The facility had cleansing wipes and they will be used to prepare the clients' skin before waxing.

- Lash brushes and a shaving tool were stored in the disinfectant tray. An operator was observed cleaning these single use items.The shaving tool was placed in the sharps container. Used lash brushes were discarded. Do not reuse single use items.

- 1) There was hair in the disinfectant tray.The disinfectant tray was cleaned during the inspection. Wax strips and spools of thread for hair removal stored in the disinfectant tray were discarded.2) Hair clips and hairbrush on the storage cart were dirty with hair.Clean and disinfect hair styling tools after use.3) The handle of a hairbrush was covered with tape, which can compromise cleaning and disinfection. Replace tape with another washable material for better grip of the handle.

- 1) The high-level disinfectant expired on April 30, 2025.An operator obtained a new high-level disinfectant during the inspection. 2) Only a bathroom cleaner was used on client surfaces. A low-level disinfectant was available on-site. It will be used for client surfaces moving forward.3) There were no test strips for monitoring the high-level disinfectant during the reuse period.Provide appropriate test strips and monitor daily.4) A log for monitoring high-level disinfectant was not maintained.Maintain log daily to indicate pass/ fail result and replacement of the disinfectant.5) The operator indicated that the high-level disinfectant in the disinfectant tray was used up to 1 month. Follow the manufacturer's instructions for use. This particular disinfectant has a 14-day reuse period.6) Combs were not completely immersed in the barbicide.Submerge combs/ hair styling tools in the barbicide.7) Manufacturer's instructions were not being followed for preparing barbicide.A measuring cup was obtained during the inspection for mixing proper ratio of barbicide concentrate and water.8) Reusable equipment such as tweezers and nail clipper was placed in the disinfectant tray after cleaning without being dried first.Follow the manufacturer's instructions for use (clean, rinse, dry, disinfect, rinse then dry).9) The pedicure basin was unclean. The operator was unable to explain the procedures for cleaning and disinfecting. Clean and disinfect the pedicure basin with a low-level disinfectant at minimum after each client.

Protect equipment and supplies from contamination.2) The face bleaching product only contained key ingredients on the label.Discontinue the use of this product until a complete list of ingredients is obtained.3) Hair was found inside opened wax pots.Contaminated wax pots were discarded. Cover wax pots when not in use.

- Hair was found on many surfaces including but not limited to work surfaces, floors, equipment storage, janitorial equipment and washroom including the toilet.Conduct thorough cleaning of the entire facility.
