      • 1) The facility is using the wrong testing strips.- Please purchase the corresponding testing strips for your high level disinfectant.2) High level disinfection records are being kept on an excel spreadsheet on a tablet. However, they are missing the following required information:a) Dates when disinfectant was changed (for reuse period)b) Initials of personal services worker who performed the activity.- Please ensure you include this information in your high level disinfection logs.- Alternatively, you may choose to use the AHS log template (which will be emailed along with this report).

      • Two worker chairs have built in fabric cushions.- Please ensure the worker chair surfaces are of a material that is smooth, non-porous, and easily cleanable.- Some options include replacing these chairs or covering with a suitable material.
