      • Record of high-level disinfection test results (Pre-empt CS20) were not being kept.-Please ensure that the test results of high-level disinfectant are being kept including the following: date of tests (verification of minimum effective concentration), test trips results (pass/fail), dates when disinfectant was changed (for reuse period), and initials of personal services worker who performed the activity.

      • The needle used for microneedling did not have manufacturer's name and instructions on the packaging. -Please ensure that any sterile equipment such as needles have manufacturer's name and instructions on it or the main container box.-Please keep the main container box for manufacturer information if the packaging itself does not have it.

The top up bottle for saline (cleanser) was not cleaned, rinsed and dried before being filled up.- Ensure that the top up bottle for saline (cleanser) is refilled after the bottle has been cleaned rinsed and dried.2.The lip pigment bottles were expired.- Please make sure that the cosmetic products such as lip pigment bottles are not expired and are up to date. Please discard the products and if they are not used for the clients then put them at different location.
