      • Facility was using both preservative free and bacteriostatic saline for reconstitution of neuromodulators. The date of reconstitution was labeled, however the time was not.Going forward, please ensure that the manufacturer's instructions for use are followed, including the requirement to reconstitute only with preservative free saline, and discard any unused portion 24 hours post reconstitution.Label both the date and time of reconstitution.
