- Repeat violation May 5th 2026: Baseboard was missing in the front desk service area, resulting in exposed raw wood along the wall. Baseboards in the barber area were observed to be unsecured and temporarily fixed with tape.- Replace missing baseboards. Remove taped baseboards and repair and damaged areas.

- 1. Manicure and pedicure semi-critical tools were being disinfected using alcohol, which does not meet the requirements for high-level disinfection.- Obtain and use a high-level disinfectant with a Drug Identification Number (DIN) for the proper reprocessing of all semi-critical manicure and pedicure tools. Follow manufactures instruction for use. Obtain appropriate test strips for the disinfectant since you intend to reuse the solution, document test result and maintain records for review. 2. There was no low-level disinfectant for surfaces.- Obtain a low-level disinfectant that has a DIN for use on surfaces.3. The pedicure food bath was only being clean with soap; there was no disinfectant available for use. -Obtain a disinfectant for the pedicure foot bath.

- The nail station was observed to be very dirty. Dust accumulation was present throughout tools and work surfaces.- Thoroughly clean and sanitize the nail station, including all tools, equipment, and work surfaces. Implement routine cleaning between clients and at the end of each day to prevent dust and debris buildup.
