    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Exosomes were applied on skin after microneedling. Microneedling is not considered topical application. In order to be a cosmetic, the product must exhibit a lack of percutaneous absorption and should not have to be absorbed systemically to achieve the effect. Avari Medical products are topical. Topical use will need to apply on intact skin. Please follow manufacturer's instructions.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Plastic covers on the chairs are not smooth so they can’t be easily cleanable. Provide a smooth surface so it can be easily cleanable.

    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Cloth towel hanging on the cupboard door was used by technician throughout day to dry hands. Use a cloth towel one time or provide paper towels.
    • 20. Is reusable equipment disinfected properly?
      • 1) Alcohol was used to disinfect the tables and other hard surfaces in the facial room.Use a low level-disinfectant with a Drug Identification Number (DIN) for hard surfaces. 2) Cavicide disinfectant was expired since January 01, 2023.Follow manufacturer's instructions once it's expired, please do not use. 3) Loose clear plastic was used to cover the treatment chairs in the injection room. This plastic can be cleaned or disinfected properly.Provide a surface that can be clean and disinfect.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Bacteriostatic Sodium chloride with preservative was used to mix Botox. Follow the manufacturer's instructions and use preservative free saline solution.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Reusable glasses and cups were used to serve water and coffee. No dishwasher or 3 compartment sink available to sanitize the reusable glasses and cups. Provide single use cups or remove drink service.
