Client records must also include proper information for any laser treatments such as area of body services, frequency of laser, number of passes, and lot numbers of needles and expiry.Records must be available to PHI for at least 2 years.

      • Repeat Violation- Obtain test strips, test HLD and keep records for 2 years on site. Please obtain the appropriate test strips for the high-level disinfectant and record testing. PHI will send template.Immersion of tools must be fully submerged in HLD
