      • Staff require more training with the legislation, reprocessing, classifying equipment and requirements to operate such as leaving the sink turned on while in operation.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Single use items that have been previously used on clients observed to be with the clean reprocessed tools. Please discard single use items in the trash right away.
    • 20. Is reusable equipment disinfected properly?
      • High-level disinfectant found on site, HLD5 tested by PHI in container showed no effective disinfectant levels. If a high-level disinfectant is being used to clean semi-critical tools, you must also test solution with test strips and record testing.
    • 28. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Critical)?
      • The portable sink was found to be turned off during operation. Ensure the sink is fully functioning, including available hot water when facility is in operation. Failure to be able to provide hot water on demand will result in a closure.

      • There was no potable running water to clean operator's hand and equipment in between clients.-Please ensure adequate supply of potable water to ensure properly cleaning of hand and equipment.

      • There was no hot or cold running water available from the portable sink.-Please ensure adequate supply of hot and cold running water to ensure properly cleaning of non-critical equipment prior to disinfection step.

      • The personal service worker met onsite lacked the skills and knowledge for proper reprocessing of tools/equipment and hand hygiene.-Please train the indicated worker and ensure written procedures for hand hygiene and for tools and equipment reprocessing are available onsite.

Expired high-level disinfectant, preempt HLD5, was observed during the inspection.-Please procure and use single-use lash equipment/tools as facility is not equipped with plumbed in sink for reprocessing of semi-critical equipment such as reusable lash tweezers.2.
