    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Single use buffers and files, some visibly soiled, were observed in the reprocessing area and appeared to have been used, despite the operator stating they were clean. Single use items must be discarded after each client in accordance with the manufacturer’s instructions for use. **The single use tools observed to have been soiled were discarded at the time of inspection.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • 1. Reprocessed tools were not adequately protected from contamination during storage, as they were placed in dirty baskets. Storage baskets must be cleaned and disinfected prior to storing reprocessed instruments. **The operator was instructed to immediately reprocess the affected tools and to clean and disinfect the storage baskets or alternatively use sterile pouches to ensure tools are stored in a clean and sanitary manner.2. Nippers in the clean storage basket was observed to have debris between the nipper heads. **Clean and disinfect thoroughly. Ensure reuseable equipment is scrubbed with soap and water followed by a high-level disinfectant. Thoroughly inspect equipment before placing in clean storage unit. ***Dirty tools was removed for reprocessing at the time of the inspection.

      • Reusable high level disinfectant testing log sheet was not being maintained at the time of inspection** Please record test results and keep for review during inspections
    • 24. Are waste sharps disposed of properly?
      • The sharps container in the reprocessing room was observed to be full and overflowing beyond the designated fill line. When sharps reach the fill line, the container must be arranged for disposal. **Please safely dispose of the sharps container immediately and replace it with an empty one.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • A few nail polish containers/bottles were observed to have spill on the side. Please ensure the opened nail polish containers are clean and sanitary. **The identified containers were discarded during the inspection.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1. No disinfectant was in use to disinfect hard surfaces like client beds, chairs, and pedicure basins. Staff was using Mr. Clean cleaner only.**The operator was instructed to obtain an approved hard‑surface disinfectant to ensure effective disinfection of surfaces. Follow the Manufacture's Instruction for Use.2. The reprocessing room and the waxing room were observed to have visible dirt/debris on the floors. Please ensure both rooms are cleaned and organized to maintain a clean and sanitary environment.

      • The reprocessing area did not appear to be following a one‑way workflow for instrument cleaning. Clean items were observed in dirty areas, and dirty items were found in clean areas of the room. Please develop and implement a one‑way reprocessing workflow to prevent cross‑contamination

      • 1. Lash extension tweezer, a semi-critical equipment was being disinfected with alcohol.-Please use high-level disinfectant such as preempt CS20 to disinfect such equipment to prevent the spread of infection.2. Surface disinfectant, Flexi Clean, was expired since 2023.-Please procure effective disinfectant to ensure proper cleaning and disinfection of facility's surfaces.

      • Some cosmetic containers/bottles were not labeled with product names.-Please ensure all cosmetic containers or bottles are properly labeled with product names to prevent misuse.
